Description
Overview of the medicine
Lotepro-G 0.5/0.3 Suspension is an ophthalmic medication combining Loteprednol Etabonate, a corticosteroid, and Gatifloxacin, a fluoroquinolone antibiotic. It is used to treat ocular inflammation and prevent or treat associated bacterial infections in the eye.
Uses & Indications
Dosage
Adults
Instill one drop into the affected eye(s) 4 times daily, or as directed by a physician. The treatment duration should typically not exceed 7 days, unless otherwise advised.
Elderly
No specific dosage adjustment is generally required for elderly patients.
Renal_impairment
No specific dosage adjustment is required for renal impairment as systemic absorption is minimal.
How to Take
Shake well before use. Tilt head back, pull down the lower eyelid to form a pouch, and instill the prescribed number of drops. Close eye gently and apply pressure to the inner corner of the eye for a minute to prevent drainage. Avoid touching the dropper tip to any surface.
Mechanism of Action
Loteprednol Etabonate acts as a corticosteroid by inhibiting inflammatory mediators, thereby reducing swelling, redness, and discomfort. Gatifloxacin is a broad-spectrum fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Anti-inflammatory and antimicrobial effects can be observed within hours to days.
Excretion
Metabolites of Loteprednol Etabonate are excreted primarily via the kidney. Gatifloxacin is excreted mainly via the kidney.
Half life
Loteprednol Etabonate is rapidly metabolized; systemic half-life is short. Gatifloxacin ophthalmic solution half-life is not well-quantified systemically due to minimal absorption but has sustained local presence.
Absorption
Systemic absorption of Loteprednol Etabonate is minimal following ocular administration. Gatifloxacin achieves high concentrations in ocular tissues, with minimal systemic absorption.
Metabolism
Loteprednol Etabonate is rapidly metabolized to inactive metabolites upon ocular absorption. Gatifloxacin is primarily eliminated unchanged.
Side Effects
Contraindications
- Viral diseases of the cornea and conjunctiva (e.g., epithelial herpes simplex keratitis, vaccinia, varicella).
- Mycobacterial infection of the eye.
- Fungal diseases of ocular structures.
- Hypersensitivity to any component of the medication.
Drug Interactions
Other topical ophthalmic medications
If using more than one topical ophthalmic product, administer them at least 5 minutes apart.
Storage
Store at controlled room temperature (20°C to 25°C). Protect from light. Do not freeze.
Overdose
Overdose with ophthalmic solutions is unlikely to cause systemic toxicity due to minimal absorption. If accidental ingestion occurs, seek medical advice.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Exercise caution when administered to a nursing woman. Consult a physician.
