Lotrel-05-suspension is a hypothetical or custom-compounded oral liquid medication containing Amlodipine (a calcium channel blocker) and Benazepril (an ACE inhibitor). It is used to treat high blood pressure by relaxing blood vessels and reducing the heart's workload.
Uses & Indications
Dosage
Adults
Individualized, typically 5 mL orally once daily, adjusted based on patient response and tolerability. Max Amlodipine 10 mg/day and Benazepril 40 mg/day.
Elderly
Lower initial doses may be considered, and dose titration should be gradual due to potential for greater sensitivity.
Renal_impairment
Initial dose adjustment may be necessary for patients with severe renal impairment (creatinine clearance < 30 mL/min). Monitor renal function.
How to Take
Take orally once daily, with or without food. Shake the suspension well before each use.
Mechanism of Action
Amlodipine, a dihydropyridine calcium channel blocker, inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle, causing peripheral vasodilation. Benazepril, an ACE inhibitor, prevents the conversion of angiotensin I to angiotensin II, leading to decreased vasoconstriction and aldosterone secretion, thus reducing blood pressure.

Excretion
Amlodipine: Primarily excreted in urine as inactive metabolites (60%). Benazeprilat: Excreted via kidney (urine) and bile.
Half life
Amlodipine: 30-50 hours. Benazeprilat: 10-11 hours.
Absorption
Amlodipine is slowly and well absorbed from the GI tract (bioavailability 64-90%). Benazepril is rapidly absorbed and extensively metabolized to its active metabolite, benazeprilat (bioavailability of benazeprilat is about 37%).
Metabolism
Amlodipine: Extensively metabolized in the liver to inactive metabolites. Benazepril: Primarily hepatic conversion to benazeprilat.
Side Effects
Contraindications
- •History of angioedema related to previous ACE inhibitor treatment.
- •Hereditary or idiopathic angioedema.
- •Severe aortic stenosis.
- •Pregnancy (especially second and third trimesters).
- •Hypersensitivity to amlodipine, benazepril, or any other ACE inhibitor.
Drug Interactions
Lithium
Increased serum lithium levels and symptoms of lithium toxicity.
CYP3A4 Inhibitors/Inducers
May affect amlodipine plasma concentrations.
Other Antihypertensive Agents
Additive hypotensive effects.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
May reduce the antihypertensive effect of benazepril and worsen renal function.
Potassium-sparing diuretics/Potassium supplements/Salt substitutes containing potassium
May lead to hyperkalemia.
Storage

Overdose
Symptoms of overdose include marked hypotension, peripheral vasodilation (with reflex tachycardia), and prolonged shock. Treatment involves symptomatic and supportive measures, including IV fluids and vasopressors for hypotension. Amlodipine is highly protein-bound, so dialysis is unlikely to be beneficial.
Pregnancy & Lactation
Category D in the 2nd and 3rd trimesters due to benazepril (fetal toxicity). Discontinue if pregnancy is detected. Amlodipine is Category C. Not recommended during lactation as benazeprilat is excreted in human milk.
