Description
Overview of the medicine
Lucazol (Omeprazole) is a proton pump inhibitor used to treat conditions caused by too much stomach acid, such as heartburn, ulcers, and GERD. It helps relieve symptoms and allows ulcers to heal.
Uses & Indications
Dosage
Adults
Typically 20 mg or 40 mg once daily, depending on the condition and severity. For H. pylori eradication, 20 mg twice daily for 7-14 days in combination with antibiotics.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is needed for patients with renal impairment.
How to Take
Take orally, usually once a day, before a meal. Swallow capsules whole; do not crush, chew, or open them.
Mechanism of Action
Omeprazole belongs to a class of antisecretory compounds, the substituted benzimidazoles, that suppress gastric acid secretion by specific inhibition of the H+/K+-ATPase enzyme system at the secretory surface of the gastric parietal cell. This system is known as the acid (proton) pump. This effect is dose-related and leads to inhibition of both basal and stimulated acid secretion irrespective of the stimulus.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Mainly excreted in urine (about 80%) and feces (about 20%).
Half life
0.5-1 hour
Absorption
Rapidly absorbed from the gastrointestinal tract, peak plasma concentration reached within 0.5-3.5 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Omeprazole, substituted benzimidazoles, or any component of the formulation
- Concomitant use with Nelfinavir
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Clopidogrel
Reduced antiplatelet effect of clopidogrel; consider alternative antiplatelet therapy or PPI.
Nelfinavir, Atazanavir
Reduced absorption of antiretroviral drugs, significantly decreasing their effectiveness. Concomitant use with nelfinavir is contraindicated.
Diazepam, Phenytoin, Citalopram
Omeprazole may inhibit the metabolism of these drugs, leading to increased plasma concentrations.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and self-limiting, including nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Use during pregnancy only if clearly needed after careful risk-benefit assessment. Omeprazole is excreted in breast milk, so caution is advised during breastfeeding.

Lab Monitoring
- Liver function tests (in patients with pre-existing hepatic impairment)
- Serum magnesium levels (with prolonged use, especially >1 year, due to risk of hypomagnesemia)
- Vitamin B12 levels (with prolonged use, >3 years, due to risk of malabsorption)
Doctor Notes
- Prescribe the lowest effective dose for the shortest duration necessary to achieve therapeutic goals.
- Counsel patients on lifestyle modifications to complement drug therapy.
- Monitor for potential long-term side effects such as hypomagnesemia, vitamin B12 deficiency, and bone health, especially in elderly patients or those on prolonged therapy.
- Review medication list for potential drug interactions, particularly with Nelfinavir, Warfarin, and Clopidogrel.
Patient Guidelines
- Take the medicine as prescribed by your doctor, usually before the first meal of the day.
- Swallow the capsule whole with water; do not crush, chew, or open it.
- Do not stop taking the medicine abruptly, even if your symptoms improve, unless advised by your doctor.

Driving Precautions
Omeprazole may cause dizziness, somnolence, and visual disturbances in some individuals. Patients should be cautioned about driving or operating machinery until they know how Omeprazole affects them.
