Description
Overview of the medicine
Calcitriol 0.25 mcg capsule is an active form of Vitamin D3, primarily used to regulate calcium and phosphate metabolism in the body. It is often prescribed for conditions such as renal osteodystrophy, hypoparathyroidism, and certain types of rickets.
Uses & Indications
Dosage
Adults
Initial: 0.25 mcg once daily, or every other day. Dosage is adjusted based on serum calcium and phosphate levels, typically 0.25-1 mcg daily.
Elderly
No specific dosage adjustment required, but close monitoring of serum calcium and phosphate is recommended.
Renal_impairment
For dialysis patients: Initial 0.25 mcg/day or every other day. Adjust based on serum calcium, phosphate, and PTH levels. Max 2 mcg/day.
How to Take
Take orally, usually once daily, with or without food. It is generally recommended to take with a meal to aid absorption.
Mechanism of Action
Calcitriol binds to and activates the vitamin D receptor (VDR) in target tissues, leading to increased intestinal absorption of calcium and phosphate, promotion of bone mineralization, and suppression of parathyroid hormone (PTH) secretion.
Pharmacokinetics
Onset
Therapeutic effects typically seen within 2-4 weeks.
Excretion
Excreted primarily in the bile and feces, with some renal excretion.
Half life
Plasma half-life is 3-6 hours; biological half-life is much longer (3-5 days).
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma levels usually reached within 3-6 hours.
Metabolism
Metabolized in the liver and kidneys by hydroxylation.
Side Effects
Contraindications
- Hypercalcemia (high levels of calcium in the blood)
- Evidence of vitamin D toxicity
- Hypersensitivity to calcitriol or any components of the formulation
Drug Interactions
Corticosteroids
Can antagonize the effects of vitamin D analogs.
Thiazide diuretics
May increase the risk of hypercalcemia.
Digitalis glycosides
Increased risk of cardiac arrhythmias if hypercalcemia occurs.
Phenobarbital, Phenytoin
May reduce calcitriol levels by increasing metabolism.
Cholestyramine, Mineral oil
May impair absorption of calcitriol.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Overdose can lead to hypercalcemia and hypercalciuria. Symptoms include nausea, vomiting, polyuria, polydipsia, weakness, headache, and dehydration. Treatment involves discontinuing calcitriol, instituting a low-calcium diet, hydration, and other supportive measures.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Calcitriol is excreted in human milk, so caution should be exercised when administered to a nursing mother.

Clinical Trials
Numerous clinical trials have established the efficacy and safety of calcitriol in treating hypocalcemia and bone disorders, particularly in chronic kidney disease patients and those with hypoparathyroidism.
Lab Monitoring
- Serum calcium (frequently, especially during dose adjustment)
- Serum phosphate
- Serum magnesium
- Alkaline phosphatase
- Parathyroid hormone (PTH) levels
- 24-hour urine calcium
- Renal function (creatinine)
Doctor Notes
- Emphasize frequent monitoring of serum calcium and phosphate levels, especially during dose initiation and adjustment.
- Educate patients on symptoms of hypercalcemia and the importance of adhering to dietary restrictions.
- Caution when co-administering with other calcium or vitamin D supplements.
Patient Guidelines
- Do not exceed the prescribed dose of calcitriol.
- Report any symptoms of hypercalcemia (e.g., nausea, vomiting, excessive thirst, frequent urination) to your doctor immediately.
- Ensure regular blood tests for calcium, phosphate, and kidney function as advised by your doctor.
- Maintain adequate hydration by drinking plenty of fluids.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Calcitriol generally does not affect the ability to drive or operate machinery. However, if you experience symptoms like dizziness or weakness due to hypercalcemia, avoid these activities.
Lifestyle Advice
- Maintain a balanced diet as recommended by your doctor, especially regarding calcium and vitamin D intake. Ensure adequate fluid intake daily. Engage in moderate physical activity if medically permitted.
