Overview of the medicine
Luvio 60 mg Capsule contains Fluoxetine, an antidepressant medication used to treat major depressive disorder, obsessive-compulsive disorder, bulimia nervosa, panic disorder, and premenstrual dysphoric disorder. It helps improve mood, sleep, appetite, and energy level and decrease anxiety and fear.
Uses & Indications
Dosage
Adults
For Bulimia Nervosa: 60 mg orally once daily. For Major Depressive Disorder, OCD, Panic Disorder: Initial 20 mg/day, gradually increased; maximum 60 mg/day. PMDD: 20 mg/day continuously or intermittently.
Elderly
Lower initial doses may be considered, usually 10-20 mg/day, with gradual titration. Maximum 60 mg/day.
Renal_impairment
No dose adjustment necessary for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Take orally once daily, with or without food. It is generally recommended to take it in the morning to avoid insomnia.
Mechanism of Action
Fluoxetine selectively inhibits the reuptake of serotonin (5-HT) in the central nervous system, thereby potentiating serotonergic activity.

Onset
Clinical effects typically observed within 2-4 weeks, though some patients may experience benefits earlier.
Excretion
Primarily excreted by the kidneys (approx. 60%) as metabolites, with a small amount in feces.
Half life

Absorption
Well absorbed orally. Peak plasma concentrations reached within 6-8 hours. Food does not affect bioavailability but may delay absorption.
Metabolism
Extensively metabolized in the liver by CYP2D6 to norfluoxetine, an active metabolite.
Side Effects
Contraindications
- Concomitant use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of discontinuing an MAOI
- Concomitant use with Thioridazine or Pimozide
- Hypersensitivity to Fluoxetine or any component of the formulation

MAOIs
Risk of serious, sometimes fatal, reactions including serotonin syndrome.
Warfarin, NSAIDs
Increased risk of bleeding due to effects on platelet function.
Thioridazine, Pimozide
Risk of QT prolongation and cardiac arrhythmias.
Tryptophan, St. John's Wort
Increased risk of serotonin syndrome.
Tricyclic Antidepressants (TCAs)
May increase plasma levels of TCAs, requiring dose adjustment.

Overdose
Symptoms include nausea, vomiting, seizures, serotonin syndrome (agitation, hallucinations, rapid heart beat, fever, overactive reflexes, nausea, vomiting, diarrhea, loss of coordination), and coma. Management involves supportive care, maintaining airway, monitoring cardiac and vital signs. Activated charcoal may be considered.

Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Fluoxetine is excreted in breast milk; caution should be exercised.
