Overview of the medicine
M-trim 400 mg Tablet is a combination antibiotic containing sulfamethoxazole and trimethoprim. It is used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
For most infections: 1 tablet (Sulfamethoxazole 400 mg / Trimethoprim 80 mg) every 12 hours for 10-14 days. For severe infections, the dose may be increased to 2 tablets every 12 hours.
Elderly
Dosage adjustment may be required based on renal function. Monitor renal function closely.
Renal_impairment
Dosage reduction is necessary. Not recommended for severe renal impairment (creatinine clearance < 15 mL/min) unless on dialysis.
How to Take
Take the tablet orally with a glass of water, with or without food. Taking it with food or milk may help to minimize gastrointestinal upset. Drink plenty of fluids to prevent crystalluria.
Mechanism of Action
Sulfamethoxazole inhibits bacterial dihydropteroate synthase, an enzyme involved in bacterial folic acid synthesis. Trimethoprim inhibits bacterial dihydrofolate reductase, an enzyme involved in the next step of folic acid synthesis. This sequential blockade provides a synergistic bactericidal effect.
Pharmacokinetics
Onset
Antimicrobial effect typically begins within 1-2 hours after administration.
Excretion
Primarily excreted by the kidneys, both by glomerular filtration and tubular secretion. A small amount is excreted in feces.
Half life
Trimethoprim: approximately 8-10 hours; Sulfamethoxazole: approximately 10-12 hours.
Absorption
Both sulfamethoxazole and trimethoprim are rapidly and extensively absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are usually reached within 1-4 hours.
Metabolism
Both components are metabolized in the liver. Sulfamethoxazole undergoes N4-acetylation and glucuronidation. Trimethoprim undergoes oxidative metabolism.
Side Effects
Contraindications
- •Hypersensitivity to sulfamethoxazole, trimethoprim, or any component of the formulation
- •Megaloblastic anemia due to folate deficiency
- •Severe renal impairment (creatinine clearance < 15 mL/min) not on dialysis
- •Severe hepatic impairment
- •Infants less than 2 months of age
- •Pregnancy at term and during lactation (due to risk of kernicterus)
Drug Interactions
Warfarin
Increased anticoagulant effect, leading to increased bleeding risk.
Phenytoin
Increased plasma concentrations of phenytoin, requiring dose adjustment.
Ciclosporin
Increased nephrotoxicity.
Methotrexate
Increased risk of methotrexate-induced hematologic toxicity.
ACE inhibitors/ARBs
Increased risk of hyperkalemia.
Diuretics (especially thiazides)
Increased risk of thrombocytopenia with purpura, particularly in elderly patients.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of acute overdose include nausea, vomiting, dizziness, headache, crystalluria, hematuria, and depression of bone marrow. Chronic overdose can lead to bone marrow suppression. Treatment is symptomatic and supportive, including gastric lavage, forced diuresis, alkalinization of urine, and possibly hemodialysis for severe cases.
Pregnancy & Lactation
Pregnancy Category C/D (depending on trimester). Avoid in late pregnancy due to the risk of kernicterus in the neonate. Use only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk; generally avoided during lactation, especially in premature or jaundiced infants, or infants with G6PD deficiency, due to kernicterus risk.
