Mantadin 100 mg Capsule contains Amantadine Hydrochloride, which is primarily used for the treatment of Parkinson's disease symptoms and drug-induced extrapyramidal reactions. It also has antiviral properties and is used for the prophylaxis and treatment of Influenza A virus infections.
Uses & Indications
Dosage
Adults
Parkinson's: 100 mg twice daily. Influenza A: 200 mg once daily or 100 mg twice daily. Maximum 400 mg/day.
Elderly
Generally lower doses, starting with 100 mg once daily, with careful titration due to potential for renal impairment and increased sensitivity.
Renal_impairment
Dose adjustment required. For CrCl 30-50 mL/min: 100 mg/day; For CrCl 15-29 mL/min: 100 mg every other day; For CrCl <15 mL/min: Not recommended or 100 mg weekly.
How to Take
Mantadin capsules should be taken orally, with or without food. It is generally recommended to take the last dose several hours before bedtime to avoid insomnia.
Mechanism of Action
Amantadine's antiparkinsonian effect is thought to be mediated by the release of dopamine from surviving dopaminergic neurons and blocking dopamine reuptake. It also has anticholinergic properties and acts as an NMDA receptor antagonist. Its antiviral action is due to interference with the function of the M2 protein, which is an ion channel specific to the influenza A virus.

Contraindications

Drug Interactions
CNS Stimulants

Thiazide Diuretics/Triamterene
May reduce amantadine clearance, leading to increased plasma levels and toxicity.
Storage

Overdose
Symptoms of overdose include cardiac arrhythmias, hypertension, seizures, agitation, hallucinations, and coma. Management involves gastric lavage, supportive care, and intravenous physostigmine for severe CNS symptoms.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Amantadine is excreted in breast milk; therefore, caution should be exercised when administered to a nursing woman.
