Description
Overview of the medicine
Mecobalamin is a vitamin B12 coenzyme, essential for nerve function, red blood cell formation, and DNA synthesis. It is used to treat vitamin B12 deficiency, various types of neuropathies (e.g., peripheral neuropathy, diabetic neuropathy), and megaloblastic anemia.
Uses & Indications
Dosage
Adults
Typically 500 mcg to 1500 mcg daily, administered as a single dose or divided into 2-3 doses. For peripheral neuropathy, 500 mcg orally three times a day is common.
Elderly
No specific dose adjustment is generally required for elderly patients, unless severe renal impairment is present.
Renal_impairment
Use with caution in severe renal impairment. Dose adjustment may be necessary if eGFR is below 30 mL/min/1.73m².
How to Take
Take orally with or without food. Some formulations can be dissolved under the tongue (sublingual) for potentially better absorption.
Mechanism of Action
As an active coenzyme of vitamin B12, Mecobalamin participates in transmethylation reactions, particularly in the synthesis of methionine from homocysteine. It is crucial for myelin sheath formation, nerve regeneration, and maintaining nerve cell health, thereby improving nerve conduction.
Pharmacokinetics
Onset
Clinical effects for chronic conditions like neuropathy may take weeks to months to become noticeable.
Excretion
Primarily excreted via urine (approximately 50-90% unchanged within 48 hours for high doses), with some biliary excretion.
Half life
Plasma half-life is complex due to extensive tissue storage, but terminal elimination half-life can range from a few days to several months.
Absorption
Well absorbed orally; peak plasma concentration typically achieved within 3-4 hours after administration. Bioavailability is about 60-70%.
Metabolism
Mecobalamin is the active form of vitamin B12 and is directly utilized by cells. It is converted to other active coenzymes within the body as needed.
Side Effects
Contraindications
- •Hypersensitivity to Mecobalamin or any component of the formulation.
- •Patients with Leber's disease (due to risk of optic nerve atrophy).
Drug Interactions
Neomycin
Can impair the absorption of vitamin B12.
Metformin
Prolonged use of metformin can decrease vitamin B12 absorption, especially at higher doses.
Colchicine
May reduce the gastrointestinal absorption of vitamin B12.
Oral Contraceptives
Long-term use may lead to reduced serum B12 levels.
Proton Pump Inhibitors (e.g., Omeprazole)
Can reduce acid production in the stomach, impairing B12 absorption over long-term use.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Mecobalamin has a very low toxicity profile, and overdose is generally not associated with serious adverse effects. Treatment is usually symptomatic and supportive, as no specific antidote exists.
Pregnancy & Lactation
Mecobalamin is generally considered safe during pregnancy and lactation at recommended doses, as vitamin B12 is essential for fetal development and is present in breast milk. However, always consult a doctor before use.
