Description
Overview of the medicine
Mefalgin is a combination medication containing Mefenamic Acid, a non-steroidal anti-inflammatory drug (NSAID), and Paracetamol, an analgesic and antipyretic. It is used to relieve pain, reduce inflammation, and lower fever.
Uses & Indications
Dosage
Adults
Typically 1 tablet (Mefenamic Acid 250 mg + Paracetamol 325 mg) 2-3 times daily, or as directed by physician. Maximum daily dose for Mefenamic Acid should not exceed 1000 mg and Paracetamol 4000 mg.
Elderly
Lower doses may be required due to increased risk of side effects and impaired renal function. Consult physician.
Renal_impairment
Use with caution; dose adjustment may be necessary. Not recommended for severe renal impairment.
How to Take
Take orally with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water. Do not crush or chew.
Mechanism of Action
Mefenamic Acid inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of inflammation, pain, and fever. Paracetamol primarily inhibits prostaglandin synthesis in the central nervous system, producing analgesic and antipyretic effects.

Excretion
Primarily excreted in urine as metabolites and unconjugated drug. A small amount is excreted in feces.
Half life
Mefenamic Acid: Approximately 2-4 hours. Paracetamol: Approximately 1.25-3 hours.
Absorption
Both Mefenamic Acid and Paracetamol are well absorbed from the gastrointestinal tract. Peak plasma concentrations for Mefenamic Acid are reached within 2-4 hours, and for Paracetamol, within 0.5-2 hours.
Metabolism
Mefenamic Acid is metabolized in the liver by cytochrome P450 enzymes (primarily CYP2C9). Paracetamol is extensively metabolized in the liver, mainly by conjugation with glucuronide and sulfate, and a small amount by CYP2E1 to a toxic metabolite, NAPQI.
Side Effects
Contraindications
- Hypersensitivity to Mefenamic Acid, Paracetamol, or other NSAIDs
- Active peptic ulcer or gastrointestinal bleeding
- Severe renal or hepatic impairment
- Severe heart failure
- Asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Third trimester of pregnancy
- Coronary artery bypass graft (CABG) surgery
Drug Interactions
Alcohol
Increased risk of liver damage with Paracetamol, increased GI side effects with Mefenamic Acid.
Cholestyramine
Reduced absorption of Paracetamol.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Lithium, Methotrexate
Increased plasma levels and toxicity.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Diuretics, ACE inhibitors, Angiotensin II receptor blockers
Reduced antihypertensive effect; increased risk of renal impairment.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, abdominal pain, drowsiness. Severe overdose, especially with Paracetamol, can lead to liver damage. Gastric lavage, activated charcoal, and specific antidotes like N-acetylcysteine for Paracetamol overdose should be administered under medical supervision.
Pregnancy & Lactation
Avoid during the third trimester of pregnancy due to risk of premature closure of the fetal ductus arteriosus. Use with caution in other trimesters if benefits outweigh risks. Mefenamic Acid and Paracetamol pass into breast milk; use with caution in lactating mothers, generally considered safe for short-term use at recommended doses.
