Uses & Indications
Dosage
Adults
Initial dose 500 mg orally twice daily with meals or 850 mg orally once daily with meals. Titrate gradually to a maximum of 2000-2550 mg daily.
Elderly
Initial dose 500 mg once daily. Dose titration should be gradual and careful due to potential decreased renal function.
Renal_impairment
Not recommended if eGFR < 30 mL/min/1.73 m². Dose adjustment required for eGFR 30-45 mL/min/1.73 m².
How to Take
Take orally with meals to reduce gastrointestinal side effects. Swallow the tablet whole; do not crush or chew.
Mechanism of Action
Metformin primarily decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. It does not stimulate insulin secretion, thus typically does not cause hypoglycemia.

Side Effects
Contraindications
- •Severe renal impairment (eGFR < 30 mL/min/1.73 m²)
- •Metabolic acidosis (including diabetic ketoacidosis)
- •Hypersensitivity to Metformin
- •Acute or chronic metabolic acidosis, including diabetic ketoacidosis, with or without coma
Drug Interactions
Cimetidine
May increase metformin concentration, increasing risk of lactic acidosis.
Corticosteroids
May increase blood glucose levels, requiring metformin dose adjustment.
Iodinated Contrast Agents
Temporarily discontinue metformin at or before the procedure and restart 48 hours later if renal function is stable.
Diuretics (thiazide, loop)
May worsen glycemic control.
Storage
Store at room temperature (20-25°C), away from moisture and light. Keep out of reach of children.
Overdose
Overdose may lead to lactic acidosis. Treatment involves supportive measures, correction of acidosis, and hemodialysis for severe cases.
Pregnancy & Lactation
Consult a doctor. Metformin is generally considered safer than other oral antidiabetics during pregnancy but insulin is often preferred. Small amounts pass into breast milk; generally considered compatible with breastfeeding with caution.
