Description
Overview of the medicine
Megafen is a combination medicine containing Ibuprofen and Paracetamol, used to relieve mild to moderate pain, inflammation, and fever. Ibuprofen is a non-steroidal anti-inflammatory drug (NSAID) that reduces inflammation and pain, while Paracetamol is an analgesic and antipyretic that helps reduce pain and fever.
Uses & Indications
Dosage
Adults
Adults: 1 tablet 2 to 3 times daily, preferably after food. Do not exceed 3 tablets in 24 hours.
Elderly
Elderly: Use the lowest effective dose for the shortest duration. Monitor for adverse effects, especially gastrointestinal.
Renal_impairment
Renal impairment: Use with caution. Dosage reduction may be necessary depending on the degree of impairment. Not recommended in severe renal failure.
How to Take
Take orally, preferably with or after food, to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water.
Mechanism of Action
Ibuprofen inhibits cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing the synthesis of prostaglandins, which are mediators of pain, inflammation, and fever. Paracetamol's mechanism is not fully understood but is thought to involve central inhibition of prostaglandin synthesis and effects on serotonergic descending pathways, leading to analgesic and antipyretic effects.
Pharmacokinetics
Onset
Onset of pain relief usually within 30-60 minutes.
Excretion
Both are primarily excreted via the kidneys as inactive metabolites. A small amount of Ibuprofen is excreted unchanged.
Half life
Ibuprofen: 1.8-3.5 hours; Paracetamol: 1-4 hours.
Absorption
Both Ibuprofen and Paracetamol are rapidly and well absorbed orally. Ibuprofen peak plasma concentration is reached in 1-2 hours; Paracetamol in 0.5-2 hours.
Metabolism
Both are primarily metabolized in the liver. Ibuprofen by oxidation and conjugation; Paracetamol mainly by conjugation with glucuronic acid and sulfate.
Side Effects
Contraindications
- Hypersensitivity to Ibuprofen, Paracetamol, or any component of the formulation
- Active peptic ulcer or gastrointestinal bleeding
- Severe heart failure, renal or hepatic impairment
- Third trimester of pregnancy
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
Drug Interactions
SSRIs
Increased risk of gastrointestinal bleeding.
Alcohol
Increased risk of hepatotoxicity with Paracetamol and gastrointestinal irritation with Ibuprofen.
Methotrexate
Increased methotrexate toxicity.
Diuretics and ACE inhibitors
Reduced efficacy of diuretics and ACE inhibitors, increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of Ibuprofen overdose may include nausea, vomiting, abdominal pain, drowsiness, and central nervous system depression. Paracetamol overdose can cause severe liver damage. Management typically involves activated charcoal if recent ingestion, supportive care, and N-acetylcysteine for paracetamol overdose.
Pregnancy & Lactation
Ibuprofen is contraindicated in the third trimester of pregnancy due to risks to the fetus. Both drugs should be used during pregnancy and lactation only if the potential benefit outweighs the potential risk. Consult a doctor.
