Meropenem is a broad-spectrum carbapenem antibiotic used to treat a wide variety of bacterial infections, including intra-abdominal infections, meningitis, and pneumonia.
Uses & Indications
Dosage
Adults
500 mg to 1 g IV every 8 hours, depending on infection severity and type.
Elderly
Dose adjustment based on renal function, similar to adults.
Renal_impairment
Dose reduction required based on creatinine clearance.
How to Take
Intravenous infusion over 15-30 minutes, or intravenous bolus injection over 3-5 minutes for doses up to 1 g.
Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, within minutes of administration.
Excretion
Mainly via urine (70% unchanged drug).
Half life
Approximately 1 hour in adults.
Absorption
Rapidly absorbed after intravenous administration.
Metabolism
Primarily hydrolyzed by dehydropeptidase-I, but meropenem is relatively stable to this enzyme. Minor metabolism.
Side Effects
Contraindications
- Hypersensitivity to meropenem or other carbapenems
- Severe hypersensitivity (e.g., anaphylactic reaction) to any other beta-lactam antibacterial agent
Drug Interactions
Probenecid
Increases meropenem plasma concentration and prolongs its half-life.
Valproic Acid
Reduced serum valproic acid levels, potentially leading to loss of seizure control.
Oral Anticoagulants
Increased anticoagulant effect when co-administered with antibiotics, including carbapenems.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
In cases of overdose, symptomatic and supportive treatment should be given. Hemodialysis can be used to remove meropenem from the body.
Pregnancy & Lactation
Use during pregnancy only if clearly needed. Excreted in breast milk, use with caution.
