Description
Overview of the medicine
Mesalamine is an anti-inflammatory drug used to treat and prevent flare-ups of ulcerative colitis and Crohn's disease.
Uses & Indications
Dosage
Adults
For Ulcerative Colitis: 800 mg three times daily (2.4 g/day) for 6 weeks. Maintenance: 1.2-2.4 g/day in divided doses.
Elderly
Use with caution; consider reduced dosage due to potential for decreased renal function.
Renal_impairment
Use with caution. Monitor renal function. Avoid if creatinine clearance <30 mL/min.
How to Take
Oral. Swallow whole with water, do not crush or chew. Can be taken with or without food.
Mechanism of Action
Mesalamine works locally in the bowel to reduce inflammation by inhibiting cyclooxygenase and lipoxygenase pathways, reducing prostaglandin and leukotriene synthesis, and scavenging free radicals.
Pharmacokinetics
Onset
Clinical improvement often seen within 3-21 days, full effect may take longer.
Excretion
Mainly excreted in feces as unchanged drug and metabolite; a smaller portion excreted renally.
Half life
Systemic half-life is short (0.5-1.5 hours for parent drug, 5-10 hours for N-acetyl-5-ASA metabolite).
Absorption
Varies depending on formulation; oral forms are generally poorly absorbed systemically, aiming for local action in the colon.
Metabolism
Undergoes N-acetylation in the liver and colonic mucosa to form N-acetyl-5-aminosalicylic acid (N-Ac-5-ASA), which is inactive.
