Mesartin Plus 5 mg Tablet is a combination medicine used to treat high blood pressure (hypertension). It contains Olmesartan, an angiotensin receptor blocker (ARB), and Amlodipine, a calcium channel blocker (CCB). This combination effectively lowers blood pressure by relaxing blood vessels and making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
Usually 1 tablet once daily, preferably at the same time each day.
Elderly
No initial dosage adjustment is generally required, but caution is advised in patients with impaired renal or hepatic function.
Renal_impairment
No initial dosage adjustment is usually required for mild to moderate renal impairment. For severe renal impairment, lower doses or alternative treatments may be considered.
How to Take
Mesartin Plus 5 mg Tablet should be taken orally with or without food. It is recommended to take it at approximately the same time each day.
Mechanism of Action
Olmesartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor in many tissues. Amlodipine is a dihydropyridine calcium antagonist (calcium ion antagonist or slow-channel blocker) that inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This reduces peripheral vascular resistance and lowers blood pressure.

Olmesartan: 2 hours. Amlodipine: 6-12 hours.

Olmesartan: Primarily via bile (50-65%) and urine (30-35%). Amlodipine: Excreted as inactive metabolites (60%) and unchanged drug (10%) in urine.

Olmesartan: 13 hours. Amlodipine: 30-50 hours.

Olmesartan: Bioavailability is approximately 26%. Amlodipine: Bioavailability is 64-90%.
Metabolism
Olmesartan: Not significantly metabolized by cytochrome P450 system. Amlodipine: Extensively metabolized by the liver to inactive metabolites via CYP3A4.

Lithium

Pregnancy: Contraindicated in the second and third trimesters due to fetal toxicity (black box warning). Discontinue if pregnancy is detected. Lactation: Not recommended. It is unknown if olmesartan is excreted in human milk. Amlodipine is excreted in human milk, but the effect on the breastfed infant is unknown.
