Description
Overview of the medicine
Metglip-Plus-25-mg-tablet is a combination medicine containing Metformin and Teneligliptin, used to improve blood sugar control in adults with type 2 diabetes mellitus.
Uses & Indications
Dosage
Adults
Usually one tablet once or twice daily, depending on the Metformin strength and physician's discretion. Typically, Metformin 500mg/1000mg + Teneligliptin 25mg once or twice daily with meals.
Elderly
Dosage should be individualized based on renal function. Close monitoring of renal function is recommended.
Renal_impairment
Not recommended for severe renal impairment (eGFR < 30 mL/min/1.73m²). Dose adjustment may be necessary for moderate impairment.
How to Take
Take orally with a meal to reduce gastrointestinal side effects, typically once or twice daily as prescribed. Swallow the tablet whole, do not crush or chew.
Mechanism of Action
Metformin decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity. Teneligliptin is a DPP-4 inhibitor that increases levels of active incretin hormones (GLP-1 and GIP), which enhance glucose-dependent insulin secretion and suppress glucagon secretion.
Pharmacokinetics
Onset
Within hours for glucose lowering.
Excretion
Metformin: Primarily excreted unchanged in urine. Teneligliptin: Excreted via both renal and hepatic routes.
Half life
Metformin: 4-9 hours. Teneligliptin: Approximately 24 hours.
Absorption
Metformin: Absolute bioavailability 50-60%. Teneligliptin: Rapidly absorbed, peak plasma concentration within 1-2 hours.
Metabolism
Metformin: Not metabolized. Teneligliptin: Primarily metabolized by CYP3A4 and FMO3.

- Acute or chronic metabolic acidosis
- Liver impairment
- Severe congestive heart failure
Drug Interactions

Increases the risk of lactic acidosis with Metformin.
Iodinated contrast agents
Temporarily discontinue Metformin before or at the time of the procedure and for 48 hours afterward.
Diuretics (especially loop diuretics)
May increase the risk of lactic acidosis.
Corticosteroids, Sympathomimetics, Thiazides
May reduce glycemic control.
Cationic drugs (e.g., Amiloride, Digoxin, Morphin, Procainamide)
May increase Metformin concentration.
