Description
Overview of the medicine
Metronidazole is an antimicrobial medication used to treat a wide variety of bacterial and parasitic infections. It belongs to the nitroimidazole class of antibiotics.
Uses & Indications
Dosage
Adults
Typical dosage for amoebiasis is 400-800 mg three times daily for 5-10 days. For trichomoniasis, a single dose of 2g or 250 mg three times daily for 7 days.
Elderly
Dosage adjustment may be necessary in elderly patients, especially with hepatic impairment.
Renal_impairment
No dosage adjustment typically required for renal impairment, but caution in severe cases.
How to Take
Oral tablets/suspension should be taken with or after food to minimize gastrointestinal upset. IV infusion is administered slowly.
Mechanism of Action
Metronidazole enters cells and is then reduced to reactive nitro-radicals by bacterial/protozoal enzymes. These radicals damage DNA and other biomolecules, leading to cell death.

Onset of action varies depending on the infection.
Excretion
Mainly renal (urine), with some fecal excretion.
Half life
Approximately 8 hours (range 6-12 hours).
Absorption
Well absorbed orally, peak plasma concentrations typically within 1-2 hours.

Primarily hepatic (liver) metabolism via oxidation and glucuronidation.
Side Effects
Contraindications

Disulfiram-like reaction (nausea, vomiting, flushing)
Lithium
Increased serum lithium levels

Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, ataxia, seizures, peripheral neuropathy. Management is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Category B. Generally avoided in the first trimester. Use with caution in later trimesters and during lactation, as it is excreted in breast milk.
