Description
Overview of the medicine
Metformin Hydrochloride 500 mg tablet is an oral antidiabetic drug used to manage blood glucose levels in patients with type 2 diabetes mellitus.
Uses & Indications
Dosage
Adults
Initial dose is usually 500 mg once or twice daily with meals. May be gradually increased to a maximum of 2000 mg/day in divided doses.
Elderly
Initiate with a lower dose, monitor renal function regularly. Maximum dose should be carefully considered.
Renal_impairment
Contraindicated if eGFR < 30 mL/min/1.73 m². Dose reduction needed if eGFR 30-45 mL/min/1.73 m².
How to Take
Take orally with meals to reduce gastrointestinal side effects. Swallow the tablet whole; do not crush or chew.
Mechanism of Action
Metformin primarily reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.
Pharmacokinetics
Onset
Within a few days, full effect in 1-2 weeks.
Excretion
Excreted unchanged in the urine.
Half life
Approximately 6.2 hours.
Absorption
Approximately 50-60% absorbed from the GI tract under fasting conditions. Food decreases the extent of absorption.
Metabolism
Not metabolized in the liver.
Side Effects
Contraindications
- Severe renal impairment (eGFR < 30 mL/min/1.73 m²)
- Acute or chronic metabolic acidosis, including diabetic ketoacidosis
- Known hypersensitivity to metformin
- Severe liver impairment
- Heart failure requiring pharmacological intervention
Drug Interactions
Alcohol
Potentiates the effect of metformin on lactate metabolism, increasing the risk of lactic acidosis.
Cimetidine
Increases metformin plasma concentration.
Iodinated contrast agents
Increased risk of lactic acidosis; discontinue metformin before or at the time of procedure and restart 48 hours later if renal function is stable.
Diuretics (thiazide, loop)
May reduce metformin efficacy.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include lactic acidosis, hypoglycemia (if combined with other antidiabetics). Management involves hemodialysis to remove metformin and supportive care.
Pregnancy & Lactation
Pregnancy Category B. Consult doctor. Small amounts may pass into breast milk; typically considered safe but use with caution and monitor infant.

YesClinical Trials
Extensive clinical trials support Metformin's efficacy and safety for type 2 diabetes. Studies like the UKPDS have demonstrated its long-term benefits.
Lab Monitoring
- Renal function (eGFR) before initiation and annually thereafter
- Vitamin B12 levels (especially in long-term use)
- HbA1c to monitor glycemic control
Doctor Notes
- Advise patients on lactic acidosis symptoms and when to seek emergency care.
- Monitor renal function and B12 levels regularly.
- Consider lifestyle modifications as cornerstone therapy.
Patient Guidelines
- Take with food to minimize stomach upset.
- Do not stop taking without consulting your doctor.
- Inform your doctor if you experience unusual muscle pain, difficulty breathing, or stomach discomfort.
- Regularly monitor blood sugar levels.
Missed Dose Advice
If a dose is missed, take it as soon as you remember unless it is almost time for the next dose. Do not double the dose to catch up.
Driving Precautions
Metformin monotherapy is not associated with hypoglycemia, so it typically does not affect driving ability. However, if used in combination with other antidiabetics that can cause hypoglycemia, or if you feel unwell, exercise caution.
Lifestyle Advice
- Follow a balanced diet and regular exercise regimen.
- Avoid excessive alcohol consumption.
- Maintain a healthy weight.
