Overview of the medicine
Metrion 500 mg Injection is a potent, broad-spectrum antibiotic used to treat various severe bacterial infections. It works by inhibiting bacterial cell wall synthesis, leading to bacterial death.
Uses & Indications
Dosage
Adults
500 mg intravenously (IV) every 8 hours. Dosage may vary based on infection severity and susceptibility.
Elderly
Dosage adjustment may be necessary in elderly patients, particularly with impaired renal function. Monitor renal function.
Renal_impairment
Reduce dosage in patients with creatinine clearance < 50 mL/min. For CrCl 20-49 mL/min, 500 mg every 12 hours. For CrCl < 20 mL/min, 250 mg every 24 hours.
How to Take
Administer slowly by intravenous infusion over 20-30 minutes. Reconstitute the powder with sterile water for injection prior to administration.
Mechanism of Action
Metrion acts by binding to penicillin-binding proteins (PBPs) in the bacterial cell wall, which are essential for cell wall synthesis. This inhibition leads to defective cell walls, increased osmotic pressure, and ultimately lysis and death of the bacterial cell.
Pharmacokinetics
Onset
Immediate onset of action after IV administration.
Excretion
Excreted predominantly by the kidneys, primarily through glomerular filtration and tubular secretion.
Half life
Approximately 1.5 to 2 hours in adults with normal renal function.
Absorption
Rapid and complete absorption following intravenous (IV) administration. Peak plasma concentrations are achieved immediately.
Metabolism
Minimal hepatic metabolism; primarily eliminated as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to Metrion or other carbapenems/beta-lactam antibiotics
- History of severe allergic reaction to beta-lactam antibiotics
Drug Interactions
Probenecid
Probenecid competitively inhibits renal tubular secretion of Metrion, resulting in increased plasma concentrations and prolonged half-life of Metrion. Concomitant administration is not recommended.
Valproic Acid
Co-administration with valproic acid may lead to decreased valproic acid concentrations and loss of seizure control. Concomitant use is generally not recommended.
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children. Reconstituted solution should be used immediately.
Overdose
In case of overdose, discontinue Metrion and provide supportive treatment. Hemodialysis can be used to remove Metrion from the circulation, especially in renally impaired patients.
Pregnancy & Lactation
Use in pregnancy only if clearly needed. Excreted in breast milk; caution advised. Consult a doctor.
