Description
Overview of the medicine
Metronidazole is an antibiotic used to treat a wide variety of bacterial and parasitic infections. It belongs to a class of drugs called nitroimidazoles. It works by stopping the growth of certain bacteria and parasites.
Uses & Indications
Dosage
Adults
For amoebiasis, 400-800 mg three times daily for 5-10 days. For trichomoniasis, 200 mg three times daily for 7 days or 2 g single dose. For anaerobic infections, 400 mg three times daily for 7 days or as directed by physician.
Elderly
Dosage adjustment may be required based on renal/hepatic function.
Renal_impairment
No dose adjustment generally needed for renal impairment unless severe.
How to Take
Take tablets orally with or after food to minimize gastrointestinal upset. Swallow whole with water; do not chew or crush.
Mechanism of Action
Metronidazole enters the bacterial/parasitic cell, is reduced to its active form, which then interacts with DNA, causing strand breakage and inhibition of nucleic acid synthesis, leading to cell death.

Varies by condition, generally within hours to days.
Excretion
Primarily excreted in urine (60-80%) as unchanged drug and metabolites; some via feces (6-15%).
Half life
Approximately 8 hours.
Absorption
Rapidly and almost completely absorbed after oral administration. Peak plasma concentrations are reached in 1-2 hours.
Metabolism
Extensively metabolized in the liver by hydroxylation, oxidation, and glucuronide conjugation.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives.
- •First trimester of pregnancy (relative contraindication).
- •Concurrent use of disulfiram within the last 2 weeks.
- •Alcohol consumption during treatment and for at least 3 days after.
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, abdominal cramps, flushing, headache, palpitations).
Lithium
May increase lithium levels, leading to toxicity.
Warfarin
May enhance anticoagulant effect.
Cimetidine
May increase metronidazole plasma levels.
Disulfiram
Psychotic reactions.
Phenobarbital
May decrease metronidazole plasma levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms: Nausea, vomiting, ataxia, disorientation, mild leukopenia, seizures. Management: No specific antidote. Symptomatic and supportive treatment. Gastric lavage may be performed.
Pregnancy & Lactation
Pregnancy: Category B in 2nd and 3rd trimesters; generally avoided in 1st trimester. Use only if clearly needed and benefits outweigh risks. Lactation: Excreted into breast milk. Advise expressing and discarding milk during treatment and for 12-24 hours after last dose.
