Description
Overview of the medicine
Mextil 125 mg Suspension is an oral third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Adults typically use higher strengths (200 mg or 400 mg tablets/capsules). This suspension is primarily for pediatric use.
Elderly
No specific dosage adjustment is necessary unless there is significant renal impairment.
Children
8 mg/kg/day, given as a single daily dose or in two divided doses every 12 hours. For example, a 15 kg child would take 120 mg/day (approx 10 ml of 125 mg/5ml suspension), often split into 5ml twice daily.
Renal_impairment
For creatinine clearance < 20 mL/min, the daily dose should be halved.
How to Take
Shake the bottle well before each use. Administer orally using a measuring spoon or oral syringe. Can be taken with or without food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan polymers, essential for bacterial cell wall strength and rigidity. This leads to the disruption of the bacterial cell wall, resulting in cell lysis and death.
Pharmacokinetics
Onset
Typically within 1-2 hours.
Excretion
Primarily excreted by the kidneys (approximately 50% of the absorbed dose) via glomerular filtration and tubular secretion. A small amount is excreted in bile.
Half life
Average plasma half-life is 3-4 hours, but may be up to 9 hours in some cases.
Absorption
Approximately 40-50% of an oral dose is absorbed from the gastrointestinal tract. Absorption is not significantly altered by food.
Metabolism
It is not extensively metabolized; about 10% of the drug is converted to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to cefixime or other cephalosporin antibiotics
- History of serious hypersensitivity reactions to penicillin
Drug Interactions
Probenecid
May increase cefixime plasma concentrations by decreasing renal excretion.
Carbamazepine
Concomitant use may increase carbamazepine levels.
Oral anticoagulants (e.g., Warfarin)
May increase prothrombin time/INR. Close monitoring of coagulation parameters is advised.
Storage
Store dry powder below 30°C, protected from light and moisture. Reconstituted suspension should be stored in a refrigerator (2-8°C) and discarded after 7-14 days.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B. Cefixime should be used during pregnancy only if clearly needed. Excreted in very small amounts in breast milk; caution should be exercised when administered to a nursing mother.
