Mic-HC 2-1 Cream is a topical medication combining miconazole nitrate, an antifungal agent, with hydrocortisone, a corticosteroid. It is used to treat fungal skin infections that are accompanied by inflammation, redness, and itching.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area twice daily, preferably in the morning and evening. Gently rub in. Treatment should not exceed 2-4 weeks.
Elderly
Same as adult dosage. Use with caution in elderly patients with thin or fragile skin.
Renal_impairment
No specific dosage adjustment required for topical application as systemic absorption is minimal.
How to Take
For external use only. Wash and dry the affected area thoroughly before application. Apply a thin layer of cream to the affected skin and surrounding area. Do not cover with occlusive dressings unless advised by a doctor.
Mechanism of Action
Miconazole nitrate inhibits the biosynthesis of ergosterol or other sterols, damaging the fungal cell membrane and altering its permeability, leading to fungal cell death. Onset

Excretion
Metabolites of hydrocortisone are excreted renally. Miconazole metabolites are excreted via urine and feces.

Not well-defined for topical application; systemic half-life of hydrocortisone is about 1.5-2 hours, miconazole is about 24 hours (systemic).
Absorption
Minimal systemic absorption through intact skin. Absorption may increase with inflamed skin or occlusive dressings. Miconazole: <1% absorbed. Hydrocortisone: <5% absorbed, but varies based on skin integrity and application area.
Metabolism
Absorbed hydrocortisone is primarily metabolized in the liver to inactive metabolites. Miconazole is extensively metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to miconazole, hydrocortisone, or any component of the cream.
- •Viral skin infections (e.g., herpes simplex, varicella).
- •Tuberculosis of the skin or other bacterial infections of the skin.
- •Rosacea or perioral dermatitis.
- •Do not use on children under 2 years of age without medical advice.
Drug Interactions
Warfarin
Although systemic absorption is minimal, prolonged use or application over large areas may theoretically increase the anticoagulant effect of warfarin. Monitor INR if used concurrently.
Other topical steroids
Avoid concurrent use of other potent topical steroids to prevent additive systemic effects.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause systemic effects due to minimal absorption. Excessive or prolonged use, especially under occlusive dressings or on large areas, can lead to local adverse effects such as skin atrophy, striae, or systemic effects of corticosteroids (e.g., Cushing's syndrome symptoms) in rare cases. Discontinue use and consult a physician.

Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus. Avoid large areas or prolonged use. Lactation: Use with caution. It is unknown if miconazole or hydrocortisone are excreted in breast milk after topical application. Consult a doctor.
