Description
Overview of the medicine
Micagin (Micafungin sodium) is an echinocandin antifungal medicine used to treat various severe fungal infections, including invasive candidiasis, candidemia, and esophageal candidiasis.
Uses & Indications
Dosage
Adults
Invasive candidiasis/candidemia: 100 mg IV once daily. Esophageal candidiasis: 150 mg IV once daily. Prophylaxis: 50 mg IV once daily.
Elderly
No dose adjustment is required based on age alone.
Renal_impairment
No dose adjustment is required for patients with renal impairment.
How to Take
Micagin should be administered as a slow intravenous infusion over a period of 1 hour.
Mechanism of Action
Micafungin inhibits the synthesis of β-(1,3)-D-glucan, a key component of the fungal cell wall, leading to osmotic instability and cell lysis. This action is fungicidal against most Candida species and fungistatic against Aspergillus.
Pharmacokinetics
Onset
Rapid onset of action following IV administration.
Excretion
Mainly excreted in feces (major route) and minimally in urine (minor route).
Half life
Approximately 14-17 hours, allowing for once-daily dosing.
Absorption
Administered intravenously, resulting in 100% systemic bioavailability.
Metabolism
Primarily metabolized in the liver via non-CYP pathways, including hydroxylation by aryl sulfatase and catechol O-methyltransferase.
Side Effects
Contraindications
- Hypersensitivity to micafungin, other echinocandins, or any component of the formulation.
Drug Interactions
Sirolimus
Micafungin may increase sirolimus exposure. Monitor sirolimus levels.
Nifedipine
Concurrent use may increase nifedipine exposure. Monitor nifedipine levels and for adverse effects.
Tacrolimus
Micafungin may decrease tacrolimus AUC. Monitor tacrolimus levels.
Cyclosporine
Coadministration with micafungin may increase cyclosporine plasma concentrations. Monitor cyclosporine levels.
Storage
Store unopened vials at controlled room temperature (20-25°C), protected from light. Reconstituted solution is stable for 24-48 hours when refrigerated.
Overdose
There is no specific antidote for micafungin overdose. Management should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution advised during lactation as it is unknown if micafungin is excreted in human milk.
