Description
Overview of the medicine
Micofun 2% w/w Oral Gel is an antifungal medication used to treat fungal infections of the mouth and throat, such as oral thrush (candidiasis).
Uses & Indications
Dosage
Adults
Apply 2.5 ml of gel four times daily after meals, holding it in the mouth for as long as possible before swallowing.
Elderly
Same as adult dosage.
Renal_impairment
No specific dose adjustment required for mild to moderate renal impairment.
How to Take
Apply the gel after meals. Hold the gel in the mouth for as long as possible (e.g., several minutes) before swallowing, allowing it to coat the affected areas. For infants and children, apply small amounts to affected areas.
Mechanism of Action
Miconazole inhibits the biosynthesis of ergosterol, a vital component of fungal cell membranes. This leads to increased cell membrane permeability and leakage of essential intracellular components, ultimately causing fungal cell death.
Pharmacokinetics
Onset
Clinical improvement usually observed within 2-3 days.
Excretion
Mainly excreted in faeces, with a small amount in urine.
Half life
Plasma half-life of miconazole is approximately 20-25 hours.
Absorption
Minimally absorbed systemically from the oral cavity.
Metabolism
Extensively metabolized in the liver (primarily by CYP2C9 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to miconazole or any excipients.
- Patients with liver dysfunction.
- Co-administration with certain drugs metabolized by CYP3A4 (e.g., warfarin, oral hypoglycemics, astemizole, terfenadine).
Drug Interactions
Warfarin
Increased anticoagulant effect, leading to bleeding risk.
Phenytoin
Increased plasma levels of phenytoin, potentially leading to toxicity.
Cyclosporine
Increased plasma levels of cyclosporine, increasing risk of adverse effects.
Oral Hypoglycemics
Enhanced hypoglycemic effect, leading to risk of hypoglycemia.
Statins (e.g., Simvastatin)
Increased risk of myopathy/rhabdomyolysis.
Storage
Store below 30°C in a dry place, away from light. Do not freeze.
Overdose
Systemic absorption is minimal, so overdose is unlikely to cause serious systemic effects. Gastric lavage may be considered if a large amount is ingested. Symptomatic and supportive measures should be taken.
Pregnancy & Lactation
Use with caution during pregnancy and lactation only if clearly needed and benefits outweigh risks. Consult a doctor.
