Description
Overview of the medicine
Miconazole nitrate is an antifungal agent used to treat skin infections such as athlete's foot, jock itch, ringworm, and other fungal infections of the skin.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area and surrounding healthy skin twice daily (morning and evening) for 2-4 weeks, or as directed by a healthcare professional.
Elderly
No specific dosage adjustment is generally required for elderly patients; use as per adult dosage.
Renal_impairment
No dosage adjustment required for topical use due to minimal systemic absorption.
How to Take
For external use only. Wash and thoroughly dry the affected area before application. Gently rub the cream into the skin until it is completely absorbed. Wash hands immediately after application.
Mechanism of Action
Miconazole inhibits the biosynthesis of ergosterol, a crucial component of fungal cell membranes. This disruption leads to increased permeability of the fungal cell membrane, leakage of essential intracellular components, and ultimately fungal cell death.
Pharmacokinetics
Onset
Symptomatic improvement typically begins within 2-3 days of treatment; however, the full course should be completed.
Excretion
Primarily excreted in feces (approximately 50%), with a smaller portion excreted in urine (approximately 10-20%), mainly as inactive metabolites.
Half life
Not clinically relevant for topical application due to minimal systemic absorption; systemically, plasma half-life is approximately 24 hours.
Absorption
Minimal systemic absorption after topical application to the skin. Less than 1% of the applied dose is absorbed systemically.
Metabolism
Systemically absorbed miconazole is extensively metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to miconazole, other imidazole derivatives, or any component of the cream.
Drug Interactions
Warfarin
Although systemic absorption is minimal, there have been rare reports of interactions with oral anticoagulants like warfarin, leading to increased anticoagulant effect and bleeding. Monitor INR if concomitant use is necessary.
Phenytoin
May increase phenytoin levels; monitor for toxicity if used concomitantly.
Storage
Store below 30°C in a dry place, protected from direct light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause systemic effects due to minimal absorption. In the event of accidental oral ingestion, supportive measures should be taken. Contact a poison control center or seek immediate medical attention.
Pregnancy & Lactation
Use during pregnancy or lactation only if the potential benefit outweighs the potential risk to the fetus or infant, and under medical supervision. Minimal systemic absorption is expected.
