Description
Overview of the medicine
Miconazole 2% w/w Oral Gel is an antifungal medication used to treat fungal infections of the mouth and throat, such as oral thrush (candidiasis). It works by inhibiting the growth of fungi.
Uses & Indications
Dosage
Adults
Apply 2.5 ml of gel four times daily after meals, holding it in the mouth for as long as possible before swallowing. Continue for at least 7 days after symptoms disappear.
Elderly
Same as adult dosage, but caution should be exercised due to potential for drug interactions.
Renal_impairment
No specific dose adjustment required as systemic absorption is minimal.
How to Take
Apply the gel to the affected areas of the mouth using a clean finger. Hold it in the mouth for as long as possible before swallowing to allow contact with the fungus. Best used after meals.
Mechanism of Action
Miconazole inhibits the biosynthesis of ergosterol, an essential component of the fungal cell membrane, leading to increased cell membrane permeability, leakage of cellular contents, and eventual fungal cell death.
Pharmacokinetics
Onset
Clinical improvement typically observed within a few days.
Excretion
Mainly via faeces (approx. 50%), with a smaller portion excreted in urine (approx. 10-20%) as inactive metabolites.
Half life
Plasma half-life is approximately 20-25 hours (systemic, but oral gel absorption is minimal).
Absorption
Minimal systemic absorption from the oral mucosa. Most remains in the mouth or is swallowed.
Metabolism
Primarily hepatic, extensively metabolized by the cytochrome P450 system (CYP2C9, CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to miconazole or any other ingredient
- •Patients with liver dysfunction (for systemic use, but caution for oral gel)
- •Concomitant use with drugs metabolized by CYP3A4 (e.g., warfarin, certain statins)
Drug Interactions
Warfarin
Increased anticoagulant effect, leading to bleeding. Monitor INR closely.
Phenytoin
Increased plasma levels of phenytoin. Monitor phenytoin levels.
Sulfonylureas
Increased hypoglycemic effect.
Statins (e.g., Simvastatin, Lovastatin)
Increased risk of myopathy/rhabdomyolysis. Avoid concomitant use.
Storage
Store below 30°C. Do not freeze. Keep out of reach of children.
Overdose
Accidental ingestion of large quantities may lead to gastrointestinal upset, nausea, vomiting, and diarrhea. Treatment is symptomatic and supportive. Gastric lavage may be considered in severe cases.
Pregnancy & Lactation
Pregnancy category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts may be excreted in breast milk; caution advised during lactation.
