Microcef-DS 200 mg Suspension contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat various bacterial infections in children.
Uses & Indications
Dosage
Adults
Not typically used in this suspension form for adults. For adults, Cefixime is usually prescribed as 200-400 mg tablet/capsule daily.
Elderly
No specific dosage adjustment needed for elderly unless there is severe renal impairment. Refer to renal impairment guidelines.
Children
8 mg/kg/day, given as a single dose or in two divided doses for 7-14 days. Maximum dose 400 mg/day.
Renal_impairment
If Creatinine Clearance is 21-60 mL/min, the dose should be reduced by 25%. If CrCl < 20 mL/min, the dose should be reduced by 50%.
How to Take
Administer orally, with or without food. Shake the suspension well before each use. Measure dose accurately using a measuring spoon or oral syringe.
Mechanism of Action
Cefixime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs) located inside the bacterial cell wall. This leads to the disruption of the cell wall, causing cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in urine via glomerular filtration and tubular secretion (about 50%), and also via biliary excretion (about 10%).
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% absorbed from the gastrointestinal tract; absorption is not significantly affected by food.
Metabolism
Not extensively metabolized; some metabolites have been identified but are not pharmacologically active.
Side Effects
Contraindications
- •Hypersensitivity to Cefixime or other cephalosporins
- •History of anaphylactic reactions to penicillin
Drug Interactions
Warfarin
May increase prothrombin time (PT) with or without bleeding, so monitor INR closely.
Probenecid
Increases Cefixime plasma concentrations by decreasing renal excretion.
Carbamazepine
Increased carbamazepine levels reported with concomitant use.
Oral anticoagulants
Enhanced anticoagulant effect.
Storage
Store dry powder below 30°C, protected from light and moisture. Keep out of reach of children. Reconstituted suspension should be stored in a refrigerator (2-8°C).
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities from the circulation by hemodialysis or peritoneal dialysis. Treatment is supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use in pregnancy only if clearly needed. Cefixime is excreted in small amounts into breast milk, use with caution during lactation.
