Description
Overview of the medicine
Midita 100 mg tablet contains Miditapram Hydrochloride, a selective serotonin reuptake inhibitor (SSRI) used to treat depression, anxiety disorders, and panic disorder. It works by increasing the level of serotonin in the brain, which helps to improve mood and reduce feelings of anxiety.
Uses & Indications
Dosage
Adults
Initial dose 50 mg once daily, preferably in the morning. May be increased to 100 mg once daily after several weeks if needed, based on clinical response and tolerability.
Elderly
A lower starting dose of 25 mg once daily may be considered. Dose titration should be gradual and carefully monitored.
Renal_impairment
No specific dose adjustment is generally required for mild to moderate renal impairment. Caution is advised in severe renal impairment (CrCl < 30 mL/min).
How to Take
Midita 100 mg tablet should be taken orally, with or without food. It is recommended to take it at the same time each day to maintain consistent drug levels.
Mechanism of Action
Miditapram selectively inhibits the reuptake of serotonin (5-HT) into presynaptic neurons, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to enhanced serotonergic neurotransmission in the central nervous system, which is believed to be responsible for its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Initial clinical improvement may be seen within 1-2 weeks, but full antidepressant effect takes 2-4 weeks.
Excretion
Mainly excreted via the kidneys (urine) and to a lesser extent through feces.
Half life
Approximately 24-36 hours.
Absorption
Well absorbed orally; peak plasma concentrations reached within 4-8 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Miditapram or any excipients.
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOIs.
- •Concomitant use with pimozide.
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes).
NSAIDs
Increased risk of gastrointestinal bleeding.
Alcohol
Should be avoided as it may potentiate central nervous system depressant effects.
Lithium
Potential for increased serotonin syndrome risk and neurotoxicity.
Triptans
Increased risk of serotonin syndrome.
Warfarin
Increased risk of bleeding due to potential enhancement of anticoagulant effects.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, somnolence, tachycardia, tremor, agitation, and dizziness. Severe overdose can lead to seizures, serotonin syndrome, and coma. Treatment is symptomatic and supportive. Maintain an open airway, monitor cardiac and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, caution should be exercised when Midita is administered to a nursing woman.
