Description
Overview of the medicine
Migranex 0.5 mg tablet is an oral medication used for the acute treatment of migraine attacks with or without aura. It belongs to the class of selective serotonin 5-HT1B/1D receptor agonists, commonly known as triptans, which work by constricting blood vessels in the brain and reducing nerve activity associated with migraine.
Uses & Indications
Dosage
Adults
The recommended adult dose is 0.5 mg as a single oral tablet at the first sign of a migraine attack. If symptoms recur, a second 0.5 mg dose may be taken after at least 2 hours. Do not exceed 1 mg in any 24-hour period.
Elderly
Use with caution in elderly patients (over 65 years). A lower initial dose or extended dosing interval may be considered, and careful monitoring for adverse effects is recommended.
Renal_impairment
No specific dose adjustment is generally required for mild to moderate renal impairment, but caution is advised. For severe renal impairment, use with caution and consider reduced dosage.
How to Take
Take Migranex 0.5 mg tablet orally, with water, as soon as possible after the onset of a migraine attack. It can be taken with or without food. Do not use for migraine prophylaxis.
Mechanism of Action
Migranex acts as a selective agonist for 5-HT1B and 5-HT1D serotonin receptors found on cranial blood vessels and nerve terminals. Activation of these receptors leads to vasoconstriction of dilated intracranial blood vessels, which is believed to contribute to migraine pain. It also inhibits the release of neuropeptides from trigeminal nerve endings, thereby reducing neurogenic inflammation and pain transmission.
Pharmacokinetics
Onset
Onset of action typically observed within 30-60 minutes.
Excretion
Excreted predominantly via the kidneys, with a smaller portion eliminated in the feces.
Half life
Elimination half-life is approximately 2-3 hours.
Absorption
Rapidly absorbed after oral administration, reaching peak plasma concentrations within 1-2 hours. Bioavailability is moderate.
Metabolism
Primarily metabolized in the liver, largely via monoamine oxidase-A (MAO-A) pathway.
Side Effects
Contraindications
- Ischemic heart disease (e.g., angina pectoris, myocardial infarction)
- History of stroke or transient ischemic attack (TIA)
- Uncontrolled hypertension
- Peripheral vascular disease
- Severe hepatic impairment
- Concomitant use with ergotamine-containing or ergot-type medications
- Concomitant use with other 5-HT1 agonists (triptans)
- Concomitant use with MAO-A inhibitors or within 2 weeks of discontinuing MAO-A inhibitors
- Hypersensitivity to the active substance or any excipients
Drug Interactions
Other 5-HT1 Agonists (Triptans)
Contraindicated. Increased risk of vasospastic reactions and adverse events. Wait at least 24 hours after discontinuing another triptan before taking Migranex, and vice versa.
Monoamine Oxidase-A (MAO-A) Inhibitors
Contraindicated. Increased systemic exposure of triptans.
Ergotamine-containing or Ergot-type Medications
Contraindicated. Increased risk of prolonged vasospastic reactions. Wait at least 24 hours after discontinuing ergotamines before taking Migranex, and vice versa.
Selective Serotonin Reuptake Inhibitors (SSRIs) and Serotonin Norepinephrine Reuptake Inhibitors (SNRIs)
Increased risk of serotonin syndrome (rare but potentially life-threatening condition) when co-administered. Monitor patients closely.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe hypertension, cardiac arrhythmias, seizures, and other cardiovascular events. In case of overdose, seek immediate medical attention. Management should be symptomatic and supportive, including monitoring of vital signs and cardiac function.
Pregnancy & Lactation
Pregnancy: Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Consult a doctor. Lactation: It is unknown whether this specific triptan is excreted in human milk. Exercise caution when Migranex is administered to a nursing woman.

Extensive clinical trials have demonstrated the efficacy and safety of Triptan Hydrochloride in the acute treatment of migraine. Data regarding Migranex 0.5 mg specifically are available from the manufacturer and regulatory submissions.
Lab Monitoring
- No routine laboratory monitoring is required for patients taking Migranex. However, a cardiovascular evaluation may be warranted prior to prescribing, especially for patients with risk factors for coronary artery disease.
Doctor Notes
- Before initiating Migranex, assess the patient's cardiovascular risk factors. Avoid use in patients with known or suspected ischemic heart disease or uncontrolled hypertension.
- Educate patients on the acute nature of Migranex treatment and the importance of not exceeding recommended doses to prevent medication overuse headache.
- Advise patients to seek immediate medical attention if they experience severe chest pain, shortness of breath, or other concerning symptoms after taking Migranex.
Patient Guidelines
- Take Migranex at the first sign of a migraine headache. It is not intended to prevent migraines.
- Do not exceed the recommended dose of 1 mg within a 24-hour period.
- If chest pain or tightness occurs and is severe or persistent, seek immediate medical attention.
- Inform your doctor about all other medications you are taking, especially other migraine treatments or antidepressants.
Missed Dose Advice
Migranex is an acute treatment for migraine, not a regular medication. Therefore, if you miss a dose, simply take it when the next migraine attack occurs. Do not take extra doses to make up for a missed one.
Driving Precautions
Migranex may cause dizziness, drowsiness, or visual disturbances. Patients should assess their reaction to the medication before driving or operating machinery.
Lifestyle Advice
- Identify and avoid known migraine triggers (e.g., certain foods, stress, lack of sleep).
