Co-trimoxazole is an antibiotic used to treat various bacterial infections. It combines two active ingredients, sulfamethoxazole and trimethoprim, which work synergistically to inhibit bacterial growth. It is commonly used for urinary tract infections, respiratory tract infections, and certain other bacterial infections.
Uses & Indications
Dosage
Adults
Standard adult dosage for Co-trimoxazole is often Sulfamethoxazole 800 mg + Trimethoprim 160 mg every 12 hours. For suspension, dosage will be adjusted based on the specific infection and patient's weight/age.
Elderly
Dosage adjustment may be required based on renal function and careful monitoring is recommended.

Significant dosage reduction or increased dosing interval required based on creatinine clearance.
How to Take
Oral, with or without food. Ensure adequate fluid intake to prevent crystalluria. Shake well before use. Follow your doctor's instructions regarding dose and duration.
Mechanism of Action
Sulfamethoxazole inhibits bacterial dihydrofolate synthesis, while trimethoprim inhibits bacterial dihydrofolate reductase. This dual blockade of folic acid metabolism leads to synergistic antibacterial action.

Side Effects
Contraindications
- •Hypersensitivity to sulfamethoxazole or trimethoprim or any component.
- •Severe renal or hepatic impairment.
- •Megaloblastic anemia due to folate deficiency.
- •Infants less than 2 months of age.
Drug Interactions
Warfarin
Increased anticoagulant effect due to inhibition of warfarin metabolism.
Methotrexate
Increased risk of methotrexate toxicity due to reduced renal excretion.
Diuretics (thiazides)
Increased risk of thrombocytopenia in elderly patients.
Potassium-sparing diuretics/ACE inhibitors
Increased risk of hyperkalemia due to trimethoprim's effect on renal potassium excretion.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Do not refrigerate after reconstitution unless specifically instructed.
Overdose
Symptoms include nausea, vomiting, dizziness, headache, mental depression, bone marrow depression. Treatment involves gastric lavage, emesis, and supportive measures. Leucovorin may be administered to counteract folate deficiency.
Pregnancy & Lactation
Pregnancy: Category C. Use only if potential benefit outweighs potential risk to the fetus. Folate supplementation may be considered. Lactation: Excreted in breast milk. Generally not recommended, especially for premature infants or those with hyperbilirubinemia, due to the risk of kernicterus.
