Description
Overview of the medicine
Mirtazapine is an antidepressant used for major depressive disorder. It helps restore the balance of natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Initially 15 mg once daily, preferably at bedtime. Dosage may be increased to a maximum of 45 mg daily.
Elderly
Similar to adults, but cautious titration recommended due to potential reduced renal/hepatic function.
Renal_impairment
Dosage adjustment may be required in moderate to severe renal impairment. Start with lower doses.
How to Take
Oral, usually once daily at bedtime due to its sedative effect. May be taken with or without food.
Mechanism of Action
Mirtazapine selectively blocks presynaptic alpha2-adrenergic autoreceptors and heteroreceptors, increasing central noradrenergic and serotonergic neurotransmission. It also blocks postsynaptic 5-HT2 and 5-HT3 receptors.
Pharmacokinetics
Onset
Antidepressant effects typically begin within 1-2 weeks, but full effect may take 4-6 weeks.
Excretion
Primarily via urine (75%) and feces (15%) as metabolites.
Half life
Approximately 20-40 hours.
Absorption
Rapidly and completely absorbed after oral administration. Peak plasma concentrations are reached within approximately 2 hours.
Metabolism
Extensively metabolized in the liver by demethylation and hydroxylation, followed by conjugation. CYP enzymes involved include CYP2D6, CYP1A2, and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to mirtazapine or any component of the formulation.
- •Concomitant use with MAOIs or within 14 days of discontinuing MAOIs.
Drug Interactions
MAOIs
Increased risk of serotonin syndrome.
CNS depressants (e.g., alcohol, benzodiazepines)
Increased CNS depression.
CYP3A4 inducers (e.g., carbamazepine, rifampicin)
Decreased mirtazapine plasma levels.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
Increased mirtazapine plasma levels.
Storage
Store below 30°C in a dry place, away from light and moisture.
Overdose
Symptoms include disorientation, drowsiness, impaired memory, tachycardia. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding.
