Overview of the medicine
Misotab (Misoprostol) is a synthetic prostaglandin E1 analogue primarily used to prevent NSAID-induced gastric ulcers. It also has significant obstetric uses including induction of labor and medical abortion.
Uses & Indications
Dosage
Adults
For NSAID-induced ulcer prevention: 200 mcg four times daily with food. For medical abortion: regimens vary, e.g., 800 mcg vaginally after mifepristone. For labor induction: 25 mcg vaginally every 3-6 hours (off-label).
Elderly
No specific dose adjustment required; however, caution is advised due to potential for increased sensitivity.
Renal_impairment
Dose reduction may be considered, especially in severe renal impairment, to avoid accumulation of the active metabolite.
How to Take
For ulcer prevention, take orally with food. For obstetric uses, it may be administered orally, sublingually, or vaginally as directed by a healthcare professional. Do not crush or chew tablets for vaginal use.
Mechanism of Action
Misoprostol binds to prostaglandin receptors on parietal cells, inhibiting gastric acid secretion. It also enhances mucosal defense by increasing bicarbonate and mucus secretion and improving mucosal blood flow. In the uterus, it causes myometrial contractions and cervical ripening.
Pharmacokinetics
Onset
Gastric protective effects begin within 30 minutes. Uterine effects vary by route and dose.
Excretion
Primarily excreted in the urine (approximately 80%) and to a lesser extent in the feces.
Half life
Misoprostol acid: approximately 20-40 minutes.
Absorption
Rapidly absorbed after oral administration and undergoes rapid de-esterification to its active metabolite, misoprostol acid. Bioavailability is reduced by food but may reduce incidence of diarrhea.
Metabolism
Undergoes extensive first-pass metabolism. Rapidly and extensively metabolized via fatty acid oxidation system to various oxidized and reduced forms.
Side Effects
Contraindications
- •Known hypersensitivity to misoprostol or other prostaglandins.
- •Pregnancy (except when used for medical abortion or labor induction under strict medical supervision).
- •In women of childbearing potential, if not using effective contraception for ulcer prevention.
Drug Interactions
Oxytocin
Increased risk of uterine hyperstimulation when used concurrently for labor induction.
Other uterine stimulants
Increased risk of adverse uterine effects.
Antacids containing magnesium
May exacerbate diarrhea.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include sedation, tremor, convulsions, dyspnea, abdominal pain, diarrhea, fever, palpitations, hypotension, and bradycardia. Treatment is supportive and symptomatic; there is no specific antidote.
Pregnancy & Lactation
Highly contraindicated in pregnancy unless its use is specifically for termination of pregnancy or induction of labor under medical supervision. It is excreted in breast milk; use with caution during lactation, or consider alternative feeding methods.
