Description
Overview of the medicine
Mometasone Furoate XR is an extended-release synthetic corticosteroid nasal spray, primarily used to treat and prevent seasonal and perennial allergic rhinitis symptoms. Its 24-hour action helps provide continuous relief from nasal congestion, sneezing, runny nose, and itching.
Uses & Indications
Dosage
Adults
Typically two sprays (50 mcg/spray) in each nostril once daily. Dosage may be reduced to one spray in each nostril once daily after symptoms are controlled.
Elderly
No specific dosage adjustment required, but use with caution due to potential age-related organ function decline.
Renal_impairment
No specific dosage adjustment required for renal impairment.
How to Take
For intranasal use only. Shake well before each use. Prime the pump before first use or if not used for a long time. Clear nasal passages before administration. Tilt head slightly forward and insert nozzle into nostril, spray while inhaling gently. Repeat for other nostril.
Mechanism of Action
Mometasone furoate is a potent glucocorticoid that binds to glucocorticoid receptors, leading to transcription of anti-inflammatory genes and inhibition of pro-inflammatory mediators (e.g., prostaglandins, leukotrienes, cytokines). This results in reduced inflammation and allergic responses in the nasal passages.
Pharmacokinetics
Onset
Onset of action typically observed within several hours, with full therapeutic effect sometimes taking up to 1-2 weeks.
Excretion
Excreted primarily in the feces, with a minor amount in the urine.
Half life
Plasma elimination half-life is approximately 5-8 hours for systemic mometasone, but systemic exposure from nasal spray is very low.
Absorption
Systemic absorption is minimal (less than 1%) following intranasal administration, primarily due to low oral bioavailability of swallowed portion.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to mometasone furoate or any excipients
- •Untreated localized infection in the nasal passages (e.g., herpes simplex)
Drug Interactions
Ketoconazole
Co-administration with potent CYP3A4 inhibitors (e.g., ketoconazole) may increase systemic exposure to mometasone. Use with caution.
Other Corticosteroids
Concurrent use with other corticosteroids (oral, inhaled, topical) may increase the risk of systemic corticosteroid side effects.
Storage
Store below 30°C, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose is unlikely to cause severe effects due to low systemic absorption. Chronic overdose or misuse can lead to hypercorticism or adrenal suppression, especially with prolonged high-dose use. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution advised during lactation as corticosteroids are excreted in breast milk, though systemic absorption from nasal spray is low.
