Description
Overview of the medicine
Monasic 10 mg tablet contains Monacic Acid, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, reduce inflammation, and lower fever. It is prescribed for conditions such as osteoarthritis, rheumatoid arthritis, muscle pain, and menstrual pain.
Uses & Indications
Dosage
Adults
10 mg orally, two to three times daily, preferably after meals. Maximum daily dose should not exceed 30 mg.
Elderly
Lower doses may be required, generally 10 mg once or twice daily, due to increased risk of adverse effects and reduced renal function. Close monitoring is recommended.
Renal_impairment
Use with caution. For moderate impairment, consider reducing the dose to 10 mg once daily. Not recommended in severe renal impairment (CrCl < 30 mL/min).
Hepatic_impairment
Use with caution. Lower doses may be necessary. Not recommended in severe hepatic impairment.
How to Take
Take the tablet orally, preferably with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water, do not chew or crush it.
Mechanism of Action
Monacic Acid works by inhibiting the synthesis of prostaglandins, which are chemicals in the body that contribute to pain, inflammation, and fever. It primarily achieves this by inhibiting cyclooxygenase (COX) enzymes, particularly COX-1 and COX-2.
Pharmacokinetics
Onset
Pain relief usually starts within 30-60 minutes.
Excretion
Mainly excreted through the urine (60-70%) as metabolites, with a smaller portion (30-40%) excreted in feces via bile.
Half life
Approximately 2-4 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 1-2 hours after oral administration.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (e.g., CYP2C9) to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Monacic Acid or other NSAIDs
- Active gastrointestinal bleeding or peptic ulcer
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- History of asthma, urticaria, or allergic reactions after taking aspirin or other NSAIDs
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
Reduced efficacy of these drugs and increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
SSRIs (Selective Serotonin Reuptake Inhibitors)
Increased risk of gastrointestinal bleeding.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. In severe cases, respiratory depression, coma, and acute renal failure may occur. Management involves symptomatic and supportive care. Gastric lavage and activated charcoal may be considered within an hour of ingestion.
Pregnancy & Lactation
Monasic Acid is contraindicated during the third trimester of pregnancy due to the risk of premature closure of the fetal ductus arteriosus and potential renal dysfunction. In early pregnancy and during lactation, use only if clearly needed and the potential benefit outweighs the risk. Consult a doctor.
