Description
Overview of the medicine
Monipro-250-mg-suspension contains Amoxicillin, a penicillin-type antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria. The 250 mg/5 ml suspension is commonly prescribed for children.
Uses & Indications
Dosage
Adults
Not typically used as 250 mg suspension for adults. If prescribed, dosage varies greatly depending on infection severity and type, usually 250 mg to 500 mg every 8 hours, or 500 mg to 875 mg every 12 hours.
Elderly
Dosage adjustment may be required based on renal function; monitor kidney function.
Children
Children (under 40 kg): 20-50 mg/kg/day in divided doses every 8-12 hours, depending on infection and weight. For 250 mg/5 ml suspension: A common dose for a child (e.g., 10-20 kg) is 125 mg (2.5 ml) to 250 mg (5 ml) three times daily.
Renal_impairment
Dosage adjustment is necessary. For mild to moderate impairment (CrCl >30 mL/min), no significant change. For severe impairment (CrCl <30 mL/min), increase dosing interval or reduce dose. Consult specific guidelines.
How to Take
For oral administration. Shake the bottle well before each use. Can be taken with or without food. Use the provided measuring cup or spoon for accurate dosing.
Mechanism of Action
Amoxicillin is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Typically within 1 hour after administration.
Excretion
Primarily excreted unchanged in the urine, predominantly by glomerular filtration and tubular secretion. About 60-70% of the dose is excreted within 6 hours.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function; prolonged in patients with renal impairment.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, even in the presence of food. Peak plasma concentrations are usually achieved within 1-2 hours after oral administration.
Metabolism
Approximately 10-25% of amoxicillin is metabolized to inactive penicilloic acid in the liver.
Side Effects
Contraindications
- Known hypersensitivity to amoxicillin, any other penicillin, or any component of the formulation.
- History of severe allergic reaction (e.g., anaphylaxis, Stevens-Johnson syndrome) to any beta-lactam antibiotic (e.g., cephalosporins, carbapenems).
Drug Interactions
Probenecid
Increases and prolongs amoxicillin blood levels by decreasing renal tubular secretion.
Allopurinol
Concomitant use may increase the incidence of skin rash.
Methotrexate
May increase methotrexate toxicity due to reduced renal excretion.
Oral Contraceptives
Amoxicillin may reduce the efficacy of oral contraceptives; advise patients to use an additional non-hormonal method of contraception.
Warfarin and other oral anticoagulants
May prolong prothrombin time; monitor INR closely.
Storage
Store the dry powder at room temperature, below 30°C, in a cool, dry place, protected from moisture. Keep out of reach of children. Store the reconstituted suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
Symptoms of overdose may include gastrointestinal disturbances (nausea, vomiting, diarrhea), fluid and electrolyte imbalance. In rare cases, crystalluria leading to renal failure has been observed. Treatment is supportive; hemodialysis may be beneficial in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe during pregnancy. Amoxicillin is excreted in small amounts in breast milk; generally considered compatible with breastfeeding, but monitor infant for potential adverse effects such as diarrhea, candidiasis, or skin rash.

YesClinical Trials
Amoxicillin has been subject to extensive clinical research and trials since its introduction, establishing its broad-spectrum efficacy and safety in treating various bacterial infections. Ongoing research continues to monitor resistance patterns and optimal dosing strategies.
Lab Monitoring
- For prolonged therapy, periodic assessment of renal, hepatic, and hematopoietic function is recommended.
- Complete Blood Count (CBC) in some cases.
Doctor Notes
- Emphasize the importance of completing the full course to prevent resistance.
- Inquire about penicillin allergy history before prescribing.
- Advise parents on proper reconstitution and administration techniques for pediatric patients.
- Monitor for signs of superinfection, especially C. difficile-associated diarrhea.
Patient Guidelines
- Complete the entire course of medication as prescribed, even if you start feeling better, to prevent recurrence and antibiotic resistance.
- Do not share this medicine with others.
- Report any unusual or severe side effects to your doctor immediately, especially allergic reactions.
- Shake the suspension well before each use and use the provided measuring device.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Amoxicillin generally does not impair the ability to drive or operate machinery. However, if side effects such as dizziness or confusion occur, patients should avoid such activities.
Lifestyle Advice
- Maintain good personal hygiene to prevent the spread of infection.
- Ensure adequate fluid intake during treatment.
- Avoid self-medication for future infections; always consult a doctor.
