Description
Overview of the medicine
Montelukast is a leukotriene receptor antagonist used for the prevention and long-term treatment of asthma and the relief of symptoms of allergic rhinitis. It works by blocking substances in the body called leukotrienes, which cause narrowing and swelling of airways in the lungs and allergy symptoms.
Uses & Indications
Dosage
Adults
For adults and adolescents 15 years and older, the recommended dose is one 10 mg tablet daily, taken in the evening. (Note: This specific product is 5mg, so it's primarily for children as per typical Montelukast dosing guidelines for 5mg ODT)
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.

How to Take
Montair-ODT should be placed on the tongue, allowed to disintegrate, and then swallowed with saliva. It should not be chewed. It can be taken with or without food, preferably in the evening.
Mechanism of Action
Montelukast selectively blocks the CysLT1 receptor, preventing the binding of cysteinyl leukotrienes (LTC4, LTD4, LTE4). This inhibition reduces bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment, which are key components of asthma and allergic rhinitis.
Pharmacokinetics
Onset
Clinical improvement typically seen within 1 day for chronic asthma and allergic rhinitis. For exercise-induced bronchoconstriction, protection is observed within 2 hours after a single dose.
Excretion
Primarily excreted in the feces via bile (approximately 86% of the dose within 5 days). Less than 0.2% is excreted in the urine.
Half life
Plasma half-life ranges from 2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Tmax) achieved in approximately 2-4 hours. Bioavailability is about 64%.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzymes CYP3A4, CYP2C9, and CYP2C8.
Side Effects
Contraindications
- •Hypersensitivity to montelukast or any component of the formulation.
- •Acute asthma attacks (Montair-ODT is not indicated for the reversal of bronchospasm in acute asthma attacks).
Drug Interactions
Rifampin
Coadministration with rifampin, a potent inducer of CYP3A4, CYP2C8, and CYP2C9, significantly decreases the plasma concentration of montelukast. Clinical monitoring is advised.
Phenobarbital
Coadministration with phenobarbital may decrease the area under the plasma concentration-time curve (AUC) of montelukast by approximately 40%, indicating that a dosage adjustment of montelukast may be necessary.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In cases of overdose, symptoms are generally mild and may include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is supportive and symptomatic, as there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Montelukast is excreted in human breast milk; use with caution in nursing mothers.
