Description
Overview of the medicine
Montelukast is an oral medication that helps to prevent and treat chronic asthma and to relieve symptoms of seasonal and perennial allergic rhinitis. It works by blocking substances in the body called leukotrienes, which contribute to asthma and allergy symptoms.
Uses & Indications
Dosage
Adults
Generally 10 mg once daily in the evening. For 5 mg tablets, if prescribed for adults, it's usually for specific clinical conditions or off-label use.
Elderly
No dosage adjustment generally required.
Renal_impairment
No dosage adjustment required.
Children_6_14_years
5 mg once daily, taken in the evening.
How to Take
Take once daily in the evening, with or without food. For ODTs, place on the tongue and allow it to dissolve completely before swallowing with or without water.
Mechanism of Action
Montelukast is a selective and orally active leukotriene receptor antagonist that specifically binds to the CysLT1 receptor. By blocking the action of leukotrienes (LTC4, LTD4, LTE4), it inhibits bronchoconstriction, reduces airway inflammation, and prevents edema and mucus secretion in the airways.
Pharmacokinetics
Onset
Improvement in asthma control usually within 1 day.
Excretion
Primarily excreted in feces (86%) via bile; less than 0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours in healthy young adults.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Tmax) is reached in about 2-4 hours for adults, 2 hours for 2-5 year olds, and 1 hour for 6-14 year olds with 5 mg ODT.
Metabolism
Extensively metabolized in the liver by CYP3A4, CYP2C9, and CYP2C8.
Side Effects
Contraindications
- •Hypersensitivity to the active substance or to any of the excipients.
- •Acute asthma attacks (not indicated for rapid relief of acute bronchospasm).
Drug Interactions
Rifampicin
May decrease montelukast plasma concentrations, possibly requiring dosage adjustment.
Gemfibrozil
Co-administration might increase montelukast exposure.
Phenobarbital
May decrease montelukast plasma concentrations.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Management is supportive. No specific antidote is known.
Pregnancy & Lactation
Pregnancy: Use only if clearly needed. Category B. Lactation: Excreted in breast milk. Use with caution.
