Overview of the medicine
Montiva (Montelukast) is a leukotriene receptor antagonist used for the maintenance treatment of asthma and to relieve symptoms of seasonal and perennial allergic rhinitis.
Uses & Indications
Dosage
Adults
10 mg orally once daily in the evening.
Elderly
No dosage adjustment generally required.
Renal_impairment
No dosage adjustment required.
How to Take
Take orally once daily in the evening, with or without food. For asthma, take in the evening. For allergic rhinitis, timing can be adjusted to patient need.
Mechanism of Action
Montelukast selectively binds to and inhibits the CysLT1 receptor, thereby blocking the actions of leukotrienes, which are inflammatory mediators involved in asthma and allergic rhinitis.
Pharmacokinetics
Onset
Within 1 day for asthma, within hours for allergic rhinitis.
Excretion
Primarily excreted in faeces via bile (86%), less than 0.2% excreted in urine.
Half life
2.7 to 5.5 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentration reached in 2-4 hours.
Metabolism
Extensively metabolized in the liver by CYP2C8, CYP3A4, and CYP2C9.
Side Effects
Contraindications
- Hypersensitivity to the active substance or to any of the excipients
Drug Interactions
Rifampin
Strong inducer of CYP enzymes, may decrease montelukast levels.
Phenobarbital
Decreases AUC of montelukast.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms usually include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Supportive measures are indicated.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk, use with caution during lactation.
