Primarily excreted renally as rhein and its glucuronide and sulfate conjugates.

Rhein has an elimination half-life of 4-8 hours.
Absorption
Well absorbed orally, undergoes extensive first-pass metabolism to the active metabolite rhein.
Metabolism
Extensively metabolized to rhein in the liver and gut, followed by conjugation.

Clinical Trials
Extensive clinical trials have demonstrated diacerein's efficacy and safety in osteoarthritis. Ongoing post-marketing surveillance and studies continue to monitor its long-term effects.
Lab Monitoring
- Liver function tests (ALT, AST, alkaline phosphatase, bilirubin) before treatment and periodically during treatment, especially in elderly and those with hepatic impairment history.
- Renal function (creatinine clearance) in elderly patients or those with kidney problems.
Doctor Notes
- Emphasize the slow onset of action to patients to ensure compliance.
- Monitor for diarrhea, especially in the initial weeks, and advise appropriate management.
- Consider liver function tests periodically, particularly if risk factors for hepatic impairment are present.
Patient Guidelines
- Take as prescribed, with food, to improve tolerability.
- Report any severe or persistent diarrhea to your doctor.
- Do not stop taking the medicine suddenly without consulting your doctor.
- Be aware that the onset of action is slow; continue treatment even if immediate improvement is not seen.
