Description
Overview of the medicine
Moxigen-D eye drop is a combination medication containing moxifloxacin, a fluoroquinolone antibiotic, and dexamethasone, a potent corticosteroid. It is used to treat bacterial eye infections where inflammation is also present. Moxifloxacin works by killing susceptible bacteria, while dexamethasone reduces swelling, itching, and redness.
Uses & Indications
Dosage
Adults
Typically 1 drop instilled into the affected eye(s) 2-4 times daily, or as directed by a physician. Treatment duration usually 7-14 days.
Elderly
No specific dosage adjustment required.
Renal_impairment
No specific dosage adjustment required for topical ophthalmic use.
How to Take
For topical ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pouch, instil one drop, and close the eye gently. Avoid touching the dropper tip to any surface.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death. Dexamethasone is a potent glucocorticoid that suppresses inflammatory responses by inhibiting the release of inflammatory mediators, stabilizing lysosomal membranes, and reducing capillary permeability.
Pharmacokinetics
Onset
Moxifloxacin: Rapid bactericidal action (minutes to hours). Dexamethasone: Anti-inflammatory effects within hours.
Excretion
Systemic excretion mainly via feces and urine (minimal for ophthalmic use); ocular elimination is local.
Half life
Moxifloxacin: Systemic plasma half-life ~11-14 hours (minimal systemic exposure). Dexamethasone: Systemic biological half-life ~36-54 hours (minimal systemic exposure).
Absorption
Limited systemic absorption following topical ophthalmic administration; adequate local absorption in ocular tissues.
Metabolism
Primarily hepatic (for systemically absorbed drug, which is minimal for ophthalmic use).
Side Effects
Contraindications
- Hypersensitivity to moxifloxacin, other quinolones, dexamethasone, or any component of the formulation.
- Viral diseases of the cornea and conjunctiva (e.g., herpes simplex keratitis, vaccinia, varicella).
- Mycobacterial eye infection.
- Fungal diseases of ocular structures.
- Untreated parasitic eye infection.
Drug Interactions
Systemic quinolones
While systemic absorption is minimal, theoretical risk of interaction exists with drugs that prolong QT interval (less relevant for topical eye drops).
Other topical corticosteroids
Increased risk of side effects.
Other topical ophthalmic medications
Administer at least 5 minutes apart.
Storage
Store at room temperature (15-30°C), protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Accidental overdose is unlikely with topical ophthalmic administration. If excess drops are applied, flush the eye with lukewarm water. Symptomatic and supportive measures if significant systemic absorption occurs (rare).
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing woman as systemic absorption is low but potential excretion in breast milk is possible. Consult a doctor.

Clinical Trials
Extensive trials have established the efficacy and safety of topical moxifloxacin and dexamethasone for ophthalmic infections and inflammation.
Lab Monitoring
- Not typically required for short-term topical ophthalmic use. For prolonged use (over 10 days), regular monitoring of intraocular pressure is recommended.
Doctor Notes
- Monitor Intraocular Pressure (IOP) if used for more than 10 days.
- Rule out viral/fungal etiology before initiation.
- Educate patients on proper instillation technique and contact lens use.
Patient Guidelines
- Complete the full course of treatment as prescribed by the doctor, even if symptoms improve.
- Do not wear contact lenses during treatment for eye infections.
- Avoid sharing eye drops to prevent spreading infection.
- If irritation persists or worsens, consult your doctor.
- Store at room temperature, away from light and moisture.
Missed Dose Advice
If a dose is missed, take it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
May cause temporary blurred vision after instillation. Patients should avoid driving or operating machinery until vision is clear.
Lifestyle Advice
- Maintain good eye hygiene.
- Avoid rubbing eyes.
- Wear protective eyewear in dusty or windy conditions.
