Overview of the medicine
Msl-Retard 15 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, swelling, and stiffness caused by arthritis. It works by blocking certain natural substances in the body that cause inflammation.
Uses & Indications
Dosage
Adults
15 mg once daily, preferably with food. Maximum recommended daily dose is 15 mg.
Elderly
Initially 7.5 mg once daily, may be increased to 15 mg once daily if needed and tolerated.
Renal_impairment
Severe renal impairment (CrCl < 30 mL/min): Contraindicated. Moderate impairment: Dose reduction may be required, monitor closely.
How to Take
Take orally once daily with water, with or after food. Do not crush, chew, or break the extended-release tablet.
Mechanism of Action
Meloxicam selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins, inflammatory mediators. By inhibiting COX-2, it reduces pain, inflammation, and fever.
Pharmacokinetics
Onset
Pain relief usually starts within 30-60 minutes, but full effect for chronic conditions may take longer.
Excretion
Approximately 50% excreted in urine and 50% in feces, primarily as inactive metabolites.
Half life
Approximately 15-20 hours.
Absorption
Well absorbed from the gastrointestinal tract, bioavailability about 89%. Food has no effect on absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9 and to a lesser extent CYP3A4.

- Hypersensitivity to Meloxicam or other NSAIDs
- Active gastrointestinal bleeding or peptic ulceration
- Severe renal or hepatic impairment
- Third trimester of pregnancy
- Coronary Artery Bypass Graft (CABG) surgery (peri-operative pain)
Drug Interactions
Lithium
Increased plasma lithium levels.
Diuretics
Decreased diuretic effect.
Methotrexate
Increased methotrexate toxicity.
Warfarin, Heparin
Increased risk of bleeding.
ACE inhibitors, Angiotensin II Receptor Blockers
Decreased antihypertensive effect, increased risk of renal impairment.

Pregnancy Category C (first and second trimester), Category D (third trimester). Avoid in the third trimester due to risk of premature closure of fetal ductus arteriosus. Use only if potential benefit outweighs risk during first and second trimesters. Not recommended during breastfeeding as it is excreted in breast milk.
