Description
Overview of the medicine
Muconil 4 mg Syrup contains Ambroxol Hydrochloride, a mucolytic agent used to liquefy and clear mucus in various respiratory conditions such as bronchitis, asthma, and emphysema.
Uses & Indications
Dosage
Adults
Adults: 5-10 ml (4-8 mg) 2-3 times daily, or as directed by the physician.
Elderly
Same as adult dose, adjust with caution if renal/hepatic impairment is present.
Renal_impairment
Dose reduction may be necessary in severe renal impairment.
How to Take
Oral administration. Can be taken with or without food. Use the provided measuring cup or spoon for accurate dosing.
Mechanism of Action
Ambroxol works by stimulating serous glandular cells, leading to the production of a less viscous mucus. It also activates mucociliary clearance by stimulating ciliary activity and decreasing mucus viscosity, making it easier to cough up and clear the airways.
Pharmacokinetics
Onset
Within 30 minutes
Excretion
Eliminated predominantly by the kidneys (about 90%) as metabolites.
Half life
Approximately 7-12 hours
Absorption
Rapidly and almost completely absorbed after oral administration. Peak plasma concentrations are reached after 1-2.5 hours.
Metabolism
Primarily metabolized in the liver to various inactive metabolites, mainly by glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to ambroxol or any component of the formulation
- Patients with rare hereditary conditions that may be incompatible with an excipient (e.g., fructose intolerance if sucrose is an excipient)
Drug Interactions
Cough suppressants
Should not be used concomitantly, as the reduction of cough reflex may lead to a dangerous accumulation of bronchial secretions.
Antibiotics (e.g., Amoxicillin, Cefuroxime, Erythromycin)
Ambroxol may increase the penetration of antibiotics into bronchial secretions, potentially enhancing their therapeutic effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and may include nausea, vomiting, diarrhea, and abdominal pain. Management is symptomatic and supportive. Gastric lavage and induced emesis are generally not recommended.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy, especially during the first trimester, should only be after careful risk-benefit assessment by a physician. Ambroxol passes into breast milk, so caution is advised during lactation.

Lab Monitoring
- No specific lab monitoring required for routine use. Monitor for signs of severe cutaneous adverse reactions (SCARs).
Doctor Notes
- Advise patients to increase fluid intake to augment the mucolytic effect.
- Caution in patients with a history of gastric ulcers due to potential mucosal irritation, although rare.
- Monitor for signs of allergic reactions or severe skin reactions, and discontinue immediately if they occur.
- Avoid co-administration with cough suppressants.
Patient Guidelines
- Do not exceed the recommended dose.
- Drink plenty of fluids to help thin mucus.
