Overview of the medicine
Multicod is a combination medication typically containing a cough suppressant (Dextromethorphan), a decongestant (Phenylephrine), and an antihistamine (Chlorpheniramine Maleate). It is used to relieve symptoms of cough, common cold, flu, and allergies such as sneezing, runny nose, watery eyes, and nasal congestion.
Uses & Indications
Dosage
Adults
10 ml (2 teaspoonfuls) every 6 hours, not to exceed 4 doses in 24 hours.
Elderly
Use with caution; consider lower doses due to potential increased sensitivity and co-morbidities.
Renal_impairment
Use with caution; dose adjustment may be necessary for severe impairment.
How to Take
Oral. Take with or without food. Use measuring spoon/cup provided.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla to suppress cough. Phenylephrine is a direct-acting sympathomimetic amine that acts as a vasoconstrictor, reducing nasal congestion. Chlorpheniramine Maleate is an alkylamine antihistamine that blocks H1 receptors, reducing sneezing, runny nose, and watery eyes.

Absorption
Dextromethorphan, Phenylephrine, and Chlorpheniramine are well absorbed orally. Dextromethorphan undergoes rapid and extensive hepatic first-pass metabolism.
Metabolism
Dextromethorphan is extensively metabolized in the liver by CYP2D6. Phenylephrine is metabolized in the liver and intestine. Chlorpheniramine is extensively metabolized in the liver.

- Hypersensitivity to any component
- Concurrent use with MAO inhibitors or within 14 days of stopping MAO inhibitors
- Severe hypertension, severe coronary artery disease
- Narrow-angle glaucoma
