Overview of the medicine
Heparin is a fast-acting anticoagulant used to prevent and treat various thromboembolic disorders, including deep vein thrombosis (DVT) and pulmonary embolism (PE). It works by enhancing the activity of antithrombin III, an enzyme that inactivates several clotting factors.

Dosage
Adults
Dosage is highly individualized based on indication, patient weight, and activated partial thromboplastin time (aPTT). Typical therapeutic IV dose: Initial bolus of 5,000 units, followed by a continuous infusion of 1,000-2,000 units/hour. Subcutaneous prophylactic dose: 5,000 units every 8-12 hours.
Elderly
May require lower doses and more frequent aPTT monitoring due to increased sensitivity and risk of bleeding. Close observation is necessary.
Renal_impairment
Use with caution. Half-life may be prolonged, requiring dose adjustment and careful monitoring of aPTT and renal function.
How to Take
Administer by continuous intravenous infusion or deep subcutaneous injection. Intramuscular administration is contraindicated due to the risk of hematoma formation.
Mechanism of Action
Heparin binds to and activates antithrombin III (ATIII), which then inactivates thrombin (Factor IIa) and Factor Xa, preventing the formation of fibrin clots. It also inhibits other clotting factors to a lesser extent.
