Description
Overview of the medicine
Mupirocin is a topical antibiotic used to treat bacterial skin infections such as impetigo and secondary infected traumatic skin lesions. It is also used for the eradication of nasal carriage of MRSA.
Uses & Indications
Dosage
Adults
Apply a small amount of ointment to the affected area 2 to 3 times daily for 3 to 10 days, or as directed by a physician. The treated area may be covered with gauze.
Elderly
No specific dosage adjustment is necessary for elderly patients.
Renal_impairment
No dosage adjustment is needed for topical administration as systemic absorption is minimal.
How to Take
For external use only. Wash hands thoroughly before and after applying the ointment. Apply a thin layer to cover the affected area. Avoid contact with eyes, nose, and mouth.
Mechanism of Action
Mupirocin inhibits bacterial protein synthesis by reversibly binding to bacterial isoleucyl-tRNA synthetase, an enzyme essential for bacterial protein synthesis. This unique mechanism of action leads to low cross-resistance with other antibiotics.
Pharmacokinetics
Onset
Clinical improvement is typically observed within 3 to 5 days of initiation of treatment.
Excretion
The inactive monic acid is primarily excreted through the urine if absorbed systemically.
Half life
The systemic half-life of mupirocin is approximately 20-40 minutes, but it is rapidly metabolized locally.
Absorption
Minimal systemic absorption occurs through intact skin. Absorption may be increased through abraded skin or open wounds.
Metabolism
Rapidly metabolized in the skin and systemically to an inactive metabolite, monic acid.
Side Effects
Contraindications
- Hypersensitivity to mupirocin or any of the excipients of the ointment.
- Not for ophthalmic or intranasal use (unless specifically formulated for nasal use).
Drug Interactions
Other topical preparations
Concurrent use of other topical preparations on the same area may dilute mupirocin and reduce its effectiveness. Avoid mixing with other topical products.
Storage
Store below 25°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Accidental ingestion of mupirocin ointment is unlikely to produce systemic toxicity due to the low systemic absorption and rapid metabolism. Topical overdose is also unlikely to cause significant systemic effects.
Pregnancy & Lactation
Pregnancy Category B. Mupirocin should be used during pregnancy only if clearly needed. It is not known whether mupirocin is excreted in human milk, but due to minimal systemic absorption, adverse effects on breastfed infants are unlikely. Use with caution during lactation.

Lab Monitoring
- No specific laboratory monitoring is generally required for topical mupirocin.
Doctor Notes
- Emphasize the importance of completing the full course of treatment to patients.
- Advise patients to avoid using mupirocin for conditions other than those prescribed.
- Monitor for signs of local irritation or allergic reactions, and discontinue use if severe reactions occur.
- Caution against use on extensive burn areas or in patients with renal impairment when large areas are treated due to polyethylene glycol base absorption.
Patient Guidelines
- Complete the full course of treatment, even if symptoms improve, to prevent recurrence and development of resistance.
- Avoid using mupirocin ointment for more than 10 days unless directed by a physician.
- Do not apply to large areas of the skin or to areas with severe burns, as this may increase systemic absorption.
- Avoid contact with eyes. If contact occurs, rinse thoroughly with water.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it is almost time for the next dose, skip the missed dose and continue with your regular dosing schedule. Do not apply a double dose to make up for a missed one.
