Description
Overview of the medicine
MV-Cod Syrup is used for the symptomatic relief of cough, cold, and allergic conditions such as sneezing, runny nose, and sore throat.
Uses & Indications
Dosage
Adults
5-10 mL orally every 4-6 hours as needed, not to exceed 4 doses in 24 hours.
Elderly
A lower dose may be required; dosage should be initiated with caution due to increased sensitivity to sedative and respiratory depressant effects.
Renal_impairment
Dose adjustment may be necessary in patients with severe renal impairment. Consult a physician.
How to Take
For oral administration. Can be taken with or without food.
Mechanism of Action
Codeine acts centrally to suppress the cough reflex by direct action on the cough center in the medulla. Promethazine is an H1-receptor antagonist that provides antihistaminic effects and has sedative properties.
Pharmacokinetics
Onset
Codeine: 30-60 minutes; Promethazine: within 20 minutes.
Excretion
Primarily excreted in the urine as metabolites and, to a lesser extent, as unchanged drug.
Half life
Codeine: 2-4 hours; Promethazine: 10-14 hours.
Absorption
Both Codeine and Promethazine are well absorbed orally. Codeine bioavailability is about 50%, Promethazine is about 25%.
Metabolism
Both are extensively metabolized in the liver. Codeine is metabolized by CYP2D6 to morphine (active metabolite) and by CYP3A4 to norcodeine. Promethazine undergoes sulfoxidation and N-demethylation.
Side Effects
Contraindications
- •Hypersensitivity to Codeine, Promethazine, or any component of the formulation.
- •Acute respiratory depression or severe asthma.
- •Children under 12 years of age (due to Codeine's risk of respiratory depression).
- •Breastfeeding mothers.
- •Patients who are ultra-rapid metabolizers of Codeine (due to increased risk of opioid toxicity).
Drug Interactions
MAO Inhibitors
Concomitant use may lead to severe adverse reactions, including hypertensive crisis or profound CNS depression. Avoid use within 14 days of MAOI discontinuation.
Anticholinergic drugs
Increased risk of anticholinergic side effects (e.g., dry mouth, urinary retention).
CNS Depressants (e.g., alcohol, sedatives, other opioids)
Increased risk of sedation, respiratory depression, and hypotension. Avoid concomitant use.
Storage
Store below 25°C in a dry place, protected from light. Keep out of reach of children.
Overdose
Symptoms of overdose include severe drowsiness, respiratory depression, pinpoint pupils, cold and clammy skin, and potentially coma. Management involves supportive care, maintaining airway and ventilation, and administration of naloxone (an opioid antagonist) if respiratory depression is severe.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy, especially in the third trimester due to risk of neonatal withdrawal syndrome. Contraindicated in breastfeeding mothers as codeine can be excreted into breast milk and cause serious adverse reactions in nursing infants, including respiratory depression.
