Description
Overview of the medicine
Mycofree-OC 0.05-0.75 Cream is a combination medicine used to treat various skin infections like dermatoses, eczema, and fungal infections, especially when complicated by bacterial or protozoal co-infections. It contains a potent corticosteroid (Clobetasol Propionate) to reduce inflammation, an antibacterial (Ofloxacin), an antiprotozoal/antibacterial (Ornidazole), and an antifungal (Terbinafine Hydrochloride) to combat a broad spectrum of pathogens.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily, or as directed by the physician. Treatment duration should generally not exceed 2 weeks.
Elderly
No specific dosage adjustments required; use with caution due to thinner skin.
Renal_impairment
No specific dosage adjustments required for topical use.
How to Take
For external use only. Wash hands before and after applying the cream. Apply a thin film to the affected skin area and rub in gently. Avoid contact with eyes, mouth, and mucous membranes. Do not use with occlusive dressings unless directed by a physician.
Mechanism of Action
Clobetasol Propionate is a potent corticosteroid that suppresses inflammatory responses. Ofloxacin is a broad-spectrum fluoroquinolone antibiotic that inhibits bacterial DNA gyrase. Ornidazole is a nitroimidazole derivative active against anaerobic bacteria and protozoa by damaging DNA. Terbinafine Hydrochloride is an allylamine antifungal that inhibits ergosterol synthesis in fungi, leading to fungal cell death.
Pharmacokinetics
Onset
Symptomatic relief usually within a few days of starting treatment.
Excretion
Absorbed components are primarily excreted renally.
Half life
Not well-defined for topical application with minimal systemic absorption. Systemic half-lives of individual components are different but not clinically relevant for topical use.
Absorption
Minimal systemic absorption after topical application; however, absorption can increase with extensive use, occlusive dressings, or application to broken skin. Clobetasol, Ofloxacin, Ornidazole, and Terbinafine are primarily active locally.
Metabolism
Minimal metabolism at the skin level; absorbed components are metabolized by the liver.
Side Effects
Contraindications
- •Hypersensitivity to Clobetasol Propionate, Ofloxacin, Ornidazole, Terbinafine Hydrochloride, or any component of the cream.
- •Viral skin infections (e.g., herpes simplex, varicella), tuberculous skin lesions, acne rosacea, perioral dermatitis, perianal and genital pruritus.
- •Not for use in children under 12 years of age.
Drug Interactions
Immunosuppressants
The local immunosuppressive effect may be enhanced.
Systemic Corticosteroids
Increased risk of systemic corticosteroid side effects if used concurrently on large areas or for prolonged periods.
CYP3A4 Inhibitors (e.g., Ritonavir, Itraconazole)
May increase systemic exposure to Clobetasol Propionate, potentially leading to Cushing's syndrome or adrenal suppression.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Do not freeze.
Overdose
Topical overdose is unlikely to cause acute systemic effects. However, prolonged or excessive use may lead to systemic effects of corticosteroids (e.g., Cushing's syndrome, adrenal suppression) or other components. Treatment is symptomatic and supportive. Discontinue use if systemic effects occur.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Topical corticosteroids should not be used extensively or for prolonged periods in pregnant patients. Exercise caution when administering to a nursing mother; it is not known whether topically administered components are excreted in human milk.
