Mycorest-50 mg Capsule contains Fluconazole, an antifungal medication used to treat a variety of fungal infections, including vaginal, oral, esophageal, systemic candidiasis, and cryptococcal meningitis.
Uses & Indications
Dosage
Adults
Dosage varies by infection type and severity. For vaginal candidiasis: 150 mg single oral dose. For oropharyngeal candidiasis: 50-200 mg once daily for 7-14 days. For systemic infections, higher doses may be required.
Elderly
Dosage adjustment may be needed based on renal function.
Renal_impairment
Dosage reduction is required for patients with impaired renal function (creatinine clearance <50 mL/min), typically 50% of the recommended dose after an initial full dose.
How to Take
Oral administration, can be taken with or without food.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450-dependent 14-alpha-demethylation of lanosterol, an essential step in fungal ergosterol biosynthesis. This leads to an accumulation of 14-alpha-methyl sterols and a decrease in ergosterol, disrupting fungal cell membrane function and growth.

Clinical effect varies with infection site and severity. Peak plasma levels reached in 1-2 hours.

Primarily renal (approx. 80% as unchanged drug).

Approximately 30 hours.
Absorption
Well absorbed orally (bioavailability >90%). Food does not significantly affect absorption. Peak plasma concentrations usually within 1-2 hours.
Metabolism
Minimally metabolized. Primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to fluconazole, other azole antifungals, or any component of the formulation.
- •Co-administration with drugs known to prolong the QT interval and metabolized by CYP3A4 (e.g., terfenadine, astemizole, cisapride, pimozide, quinidine, erythromycin).
Drug Interactions
Warfarin
Increased prothrombin time, monitor INR.
Phenytoin
Increased phenytoin levels, monitor levels.
Rifampicin
Decreased fluconazole plasma concentrations, may require increased fluconazole dose.
Oral contraceptives
Variable effects on oral contraceptive efficacy.
Cyclosporine, Tacrolimus
Increased levels of immunosuppressants, monitor levels.
Sulfonylureas (e.g., glipizide)
Increased risk of hypoglycemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hallucinations and paranoid behavior. Management involves symptomatic and supportive measures, gastric lavage, and hemodialysis may be considered.
Pregnancy & Lactation
Pregnancy Category D for long-term/high-dose use, Category C for single dose (e.g., 150 mg for vaginal candidiasis). Use only if potential benefit justifies potential risk. Fluconazole is excreted in breast milk; caution advised during lactation.
