Description
Overview of the medicine
Tropicamide is an anticholinergic agent used as an ophthalmic solution to produce mydriasis (pupil dilation) and cycloplegia (paralysis of accommodation) for diagnostic procedures of the eye.
Uses & Indications
Dosage
Adults
For refraction: Instill 1 or 2 drops of 0.5% or 1% solution into the conjunctival sac. Repeat after 5 minutes if necessary. For fundoscopy: Instill 1 or 2 drops of 0.5% solution 15-20 minutes before examination.
Elderly
Use with caution due to increased risk of systemic side effects and potential for angle-closure glaucoma.
Renal_impairment
No specific dosage adjustment provided, use with caution.
How to Take
For ophthalmic use only. Wash hands before use. Tilt head back, pull down the lower eyelid to create a pouch, and instill the prescribed number of drops without touching the eye or eyelid with the dropper tip. Close eyes gently for a few minutes.
Mechanism of Action
Tropicamide blocks the responses of the sphincter muscle of the iris and the ciliary body to cholinergic stimulation, thereby causing mydriasis and cycloplegia. It acts by competitive antagonism of acetylcholine at muscarinic receptors.
Pharmacokinetics
Onset
Mydriasis within 15-20 minutes, cycloplegia within 20-30 minutes.
Excretion
Excreted primarily via the kidneys as unchanged drug and metabolites.
Half life
Not precisely defined for ophthalmic use; effects last several hours.
Absorption
Systemic absorption occurs from the conjunctival sac, though it is generally low with topical application.
Metabolism
Undergoes hepatic metabolism to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to tropicamide or any component of the formulation.
- Patients with primary angle-closure glaucoma or a predisposition to it.
Drug Interactions
Other anticholinergic agents
Increased risk of systemic anticholinergic side effects.
Alpha-adrenergic agonists (e.g., phenylephrine)
Additive mydriatic effect.
Storage
Store at controlled room temperature (15-30°C). Protect from light. Do not freeze. Keep out of reach of children. Discard any remaining solution 4 weeks after opening.
Overdose
Systemic overdose symptoms may include flushing, dry skin, dry mouth, reduced bronchial secretions, tachycardia, arrhythmias, fever, hallucinations, confusion, and convulsions. Management is supportive, and physostigmine may be considered in severe systemic anticholinergic toxicity.
Pregnancy & Lactation
Use during pregnancy and lactation only if clearly needed and the potential benefits outweigh the risks. Consult a doctor.

YesClinical Trials
Tropicamide has been extensively studied for its mydriatic and cycloplegic effects in various clinical settings. It is a well-established ophthalmic diagnostic agent.
Lab Monitoring
- Not typically required for routine ophthalmic use.
Doctor Notes
- Always assess for risk of angle-closure glaucoma before administering mydriatics.
- Counsel patients regarding temporary visual impairment and photophobia, advising against driving.
- Monitor for systemic anticholinergic effects, especially in pediatric and elderly patients.
Patient Guidelines
- Do not touch the dropper tip to any surface to avoid contamination.
- Remove contact lenses before applying the eye drops and wait at least 15 minutes before reinserting them.
- Protect your eyes from bright light (wear sunglasses) as this medicine causes light sensitivity.
- Avoid driving or operating machinery until your vision is clear.
Missed Dose Advice
As this medicine is typically used for diagnostic purposes or before surgery, a missed dose is unlikely. If it's part of a multi-dose regimen and a dose is missed, apply it as soon as remembered, unless it's almost time for the next dose. Do not double the dose.
Driving Precautions
This medicine causes blurred vision and sensitivity to light. Do not drive or operate heavy machinery until these effects have completely subsided and your vision is clear.
Lifestyle Advice
- Wear sunglasses when outdoors to manage light sensitivity (photophobia).
- Inform your doctor if you experience any severe or persistent side effects.
