Mydrimide Plus 0.8/5 Eye Drop is a combination medication containing Tropicamide and Phenylephrine, used to dilate the pupil and paralyze the ciliary muscle for diagnostic and therapeutic ophthalmic procedures.
Uses & Indications
Dosage
Adults
1-2 drops applied to the conjunctival sac, repeated after 5-10 minutes if necessary. For cycloplegia, 1-2 drops every 5-10 minutes for 3 doses.
Elderly
Lower doses may be considered due to increased sensitivity and potential systemic side effects.
Renal_impairment
No specific dose adjustment for topical ophthalmic use, but caution is advised due to potential systemic absorption.
How to Take
For ophthalmic use only. Instill drops into the conjunctival sac. Avoid touching the dropper tip to the eye or any other surface to prevent contamination. Apply gentle pressure to the nasolacrimal duct for a minute after instillation to minimize systemic absorption.
Mechanism of Action
Tropicamide is an anticholinergic (muscarinic antagonist) that blocks the action of acetylcholine on the sphincter muscle of the iris and the ciliary body, leading to mydriasis (pupil dilation) and cycloplegia (paralysis of accommodation). Phenylephrine is an alpha-1 adrenergic agonist that acts on the dilator muscle of the iris, causing mydriasis without significant cycloplegia. The combination provides rapid and potent mydriasis and cycloplegia.

Side Effects
Contraindications

Drug Interactions
Beta-blockers
May lead to hypertension and reflex bradycardia with phenylephrine.
Other anticholinergics

Systemic sympathomimetics
Additive pressor effects.
Tricyclic antidepressants (TCAs)
May potentiate phenylephrine's pressor effects.
Monoamine oxidase inhibitors (MAOIs)
May potentiate phenylephrine's pressor effects.

Overdose
Systemic overdose symptoms may include flushing, dry skin, blurred vision, tachycardia, irregular pulse, fever, abdominal distension, convulsions, and central nervous system depression (tropicamide) or hypertension, arrhythmia, and headache (phenylephrine). Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy: Category C. Use only if potential benefit justifies potential risk to the fetus. Lactation: It is not known whether components are excreted in human milk. Caution should be exercised when administered to a nursing mother.
