Description
Overview of the medicine
N-Proton (Pantoprazole) is a proton pump inhibitor (PPI) used to treat conditions caused by excess stomach acid, such as heartburn, acid reflux, and ulcers. It works by reducing the amount of acid your stomach makes.
Uses & Indications
Dosage
Adults
Erosive Esophagitis: 40 mg once daily for up to 8 weeks. Maintenance: 40 mg once daily. Zollinger-Ellison Syndrome: Initial 40 mg twice daily, adjusted as needed.
Elderly
No dose adjustment generally required. However, caution should be exercised in elderly patients with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment or those on dialysis.
How to Take
Oral tablets should be swallowed whole with water, typically once daily before a meal (usually breakfast). Do not crush, chew, or split the tablet.
Mechanism of Action
Pantoprazole is a substituted benzimidazole that inhibits the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells. This inhibition prevents the final step in gastric acid production, leading to a sustained reduction in stomach acid secretion.
Pharmacokinetics
Onset
Onset of acid suppression within 2.5 hours. Full therapeutic effect is achieved within 2-3 days of daily dosing.
Excretion
Mainly excreted renally (approximately 80%), with the remainder eliminated in the feces via biliary excretion.
Half life
The plasma elimination half-life is approximately 1 hour, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed, peak plasma concentrations occur within 2-2.5 hours after oral administration. Bioavailability is approximately 77%. Food may delay absorption but does not affect overall bioavailability.
Metabolism
Extensively metabolized in the liver via the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to pantoprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with rilpivirine-containing products.
Drug Interactions

Antifungals (Ketoconazole, Itraconazole, Posaconazole): Reduced absorption due to increased gastric pH.

Warfarin: Increased INR and prothrombin time.

Methotrexate: Possible increased methotrexate levels and toxicity.

Clopidogrel: Potential reduction in antiplatelet effect, though clinical significance is debated and less prominent than with omeprazole.

HIV Antivirals (Atazanavir, Nelfinavir, Rilpivirine): Decreased absorption and effectiveness.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Experience with pantoprazole overdose is limited. Symptoms may include abdominal pain, diarrhea, and headache. In case of an overdose, symptomatic and supportive treatment should be administered. Pantoprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pantoprazole is excreted in human breast milk; therefore, caution should be exercised when administered to a nursing mother.
